目的：对比分析脾切除术与保脾治疗在创伤性脾破裂中的临床疗效。方法将我院收治的42例创伤性脾破裂患者随机分为脾切除组和保脾组，分别采用开腹全脾切除手术和非手术治疗或保脾手术进行治疗，对临床资料进行统计分析。结果脾切除组的下床活动时间、住院时间明显短于保脾组（P<0.05），而两者在输血量、平均住院费用、治疗有效率和并发症发生率方面对比差异无统计学意义（P>0.05）。结论对于程度较轻的创伤性脾破裂，全脾切除与保脾治疗均可获得较好的治疗效果，临床上应分析脾损伤程度，慎重选择适合的治疗方案。
목적：대비분석비절제술여보비치료재창상성비파렬중적림상료효。방법장아원수치적42례창상성비파렬환자수궤분위비절제조화보비조，분별채용개복전비절제수술화비수술치료혹보비수술진행치료，대림상자료진행통계분석。결과비절제조적하상활동시간、주원시간명현단우보비조（P<0.05），이량자재수혈량、평균주원비용、치료유효솔화병발증발생솔방면대비차이무통계학의의（P>0.05）。결론대우정도교경적창상성비파렬，전비절제여보비치료균가획득교호적치료효과，림상상응분석비손상정도，신중선택괄합적치료방안。
Objective To analyse the treatment effect of splenectomy and spleen preservation for traumatic spleen rupture comparatively. Methods 42 patients with splenic injuries were included in our hospital.They were randomly divided into splenectomy group and spleenpreservation group respectively.Splenectomy group took open splenectomy. Spleen preservation group took non-surgical methods and sleen-preserving surgery. Compare the clinical data for statistical analysis. Results Ambulation time,hospital stay of splenectomy group was significantly shorter than that of spleen preservation group (P<0.05).The differences of two gourps in total blood volume,average cost,treatment efficiency and complications is not significant (P>0.05).Conclusion Splenectomy and spleen preservation both can get good treatment effects for lesser degress traumatic spleen rupture. The damage degree of the spleen should be analysed carefully in clinical. Appropriate treatment plan should be selected cautiously.
