目的:探讨奥氮平和帕利哌酮对首发精神分裂症患者糖代谢的影响。方法:76例首发精神分裂症患者随机分为两组,分别给予奥氮平治疗(38例,奥氮平组)和帕利哌酮治疗(38例,帕利哌酮组)。两组患者治疗前和治疗后第4周末做糖耐量试验,测定空腹胰岛素、C肽、甘油三酯、胆固醇、瘦素,并测量身高、体重,计算体重指数[BMI,体重(kg)/身高(m2)]。共观察4周。结果:治疗第4周末,奥氮平组餐后1h、2h的血糖值比治疗前明显升高(t=2.47,4.75;P<0.05),而帕利哌酮无明显变化。治疗后奥氮平组(23.5%,8例)患者糖耐量减低的发生率高于帕利哌酮组(2.7%,1例),差异有显著性(χ2=4.542,P<0.05)。结论:奥氮平对首发精神分裂症患者餐后血糖值的影响大于帕利哌酮。

목적:탐토오담평화파리고동대수발정신분렬증환자당대사적영향。방법:76례수발정신분렬증환자수궤분위량조,분별급여오담평치료(38례,오담평조)화파리고동치료(38례,파리고동조)。량조환자치료전화치료후제4주말주당내량시험,측정공복이도소、C태、감유삼지、담고순、수소,병측량신고、체중,계산체중지수[BMI,체중(kg)/신고(m2)]。공관찰4주。결과:치료제4주말,오담평조찬후1h、2h적혈당치비치료전명현승고(t=2.47,4.75;P<0.05),이파리고동무명현변화。치료후오담평조(23.5%,8례)환자당내량감저적발생솔고우파리고동조(2.7%,1례),차이유현저성(χ2=4.542,P<0.05)。결론:오담평대수발정신분렬증환자찬후혈당치적영향대우파리고동。
Objective:To investigate the effects of olanzapine and paliperidone on the glucose metabo-lism in schizophrenic patients.Methods:Seventy-six first-episode schizophrenic were randomly divid-ed into two groups,which treated with olanzapine and paliperidone respectively.Before and after 4-week treatment,all the patients were given oral glucose tolerance test(OGTT),and the blood glucose values were measured.The fasting serum insulin,C-peptide,cholesterol,triglyceride,and leptin concentration were also measured.The Body Mass Index( BMI) was calculated for each patient.Results:In the OGTT, after 4-week treatment,the blood glucose at the time-points of 1 and 2 hour were higher than the base-line in olanzapine group(t=2.47,4.75;P<0.05),but unchanged in paliperidone group.The rate of im-paired glucose tolerance ( IGT ) was higher in olanzapine group than that in paliperidone group (χ2 =4.542,P<0.05).Conclusion:Olanzapine has stronger effect than paliperidone on the glucose metabo-lism.
