目的:探讨乳腺癌改良根治术中保留肋间臂神经的临床意义。方法对Ⅰ—Ⅲ期乳腺癌患者行保留肋间臂神经的改良根治术80例,与同期不保留肋间臂神经乳腺癌改良根治术96例患者的上臂内侧和腋窝区皮肤感觉功能进行观察对比随访。结果肋间臂神经保留组与肋间臂神经不保留组患肢上臂内侧及腋窝感觉异常发生率分别是11.25%及82.29%,两者比较差异有统计学意义(χ2=88.09,χ2(0.05)1=3.84,χ2>χ2(0.05)1,P<0.05)。随访6~36个月,2组均无腋窝肿瘤复发。结论在乳腺癌改良根治术中有目的地保留肋间臂神经能有效改善上臂内侧及腋窝皮肤感觉功能,从而改善患者术后的生活质量。

목적:탐토유선암개량근치술중보류륵간비신경적림상의의。방법대Ⅰ—Ⅲ기유선암환자행보류륵간비신경적개량근치술80례,여동기불보류륵간비신경유선암개량근치술96례환자적상비내측화액와구피부감각공능진행관찰대비수방。결과륵간비신경보류조여륵간비신경불보류조환지상비내측급액와감각이상발생솔분별시11.25%급82.29%,량자비교차이유통계학의의(χ2=88.09,χ2(0.05)1=3.84,χ2>χ2(0.05)1,P<0.05)。수방6~36개월,2조균무액와종류복발。결론재유선암개량근치술중유목적지보류륵간비신경능유효개선상비내측급액와피부감각공능,종이개선환자술후적생활질량。
ABSTRACT:Objective To investigate the clinical significance of preservation of intercostobra-chial nerve in modified radical mastectomy.Methods A total of 1 7 6 patients with stage Ⅰ-Ⅲbreast cancer underwent modified radical mastectomy with preservation of the intercostobrachial nerve(n=80,preservation group)or not(n=96,non-preservation group).The sensory function of upper medial arm and axillary skin were compared between the two groups.Results The inci-dence of abnormal sensation of upper medial arm and axillary skin in preservation group (11.25%)was significantly lower than that in non-preservation group(82.29%)(χ2=88.09,χ2(0.05)1=3.84,χ2>χ2(0.05)1 ,P<0.05).No axillary tumor recurrence was found in both groups dur-ing 6-3 6 months of follow-up.Conclusion The preservation of intercostobrachial nerve in modi-fied radical mastectomy can effectively improve the sensory function of upper medial arm and axil-lary skin and ameliorate the quality of life in patients with breast cancer.
