本文回顾性分析我院2011年1月～2013年11月术中发现3例医源性胆管损伤（ bile duct injury ，BDI）的临床资料。3例均术中发现并诊断，术中2例行胆管端端吻合术，1例行修补术。1例术后17个月行胆肠Roux-Y吻合术，2例术后恢复顺利。3例术后分别随访17、18、35个月，均无发热、腹痛、黄疸等症状。我们认为术中认真检查胆囊三角是降低BDI漏诊的关键。
본문회고성분석아원2011년1월～2013년11월술중발현3례의원성담관손상（ bile duct injury ，BDI）적림상자료。3례균술중발현병진단，술중2례행담관단단문합술，1례행수보술。1례술후17개월행담장Roux-Y문합술，2례술후회복순리。3례술후분별수방17、18、35개월，균무발열、복통、황달등증상。아문인위술중인진검사담낭삼각시강저BDI루진적관건。
[Summary] Clinical data of 3 cases who suffered from iatrogenic bile duct injury from January 2011 to November 2013 were collected and retrospectively analyzed . Bile duct injuries were found and diagnosed during the processes of laparoscopic cholecystectomy .Two cases were initially treated with end-to-end bile duct anastomosis , while 1 case received repair operation of bile duct surgery .Postoperatively , 1 patient received a bile-intestinal Roux-Y anastomosis after 17 months and 2 patients recovered smoothly.All the 3 cases were followed up for 17, 18, and 35 months, respectively.No postoperative complications , such as fever, abdominal pain , and jaundice , were observed .We deem that examination of the Calot ’ s triangle is the key to prevent the misdiagnosis of bile duct injury during laparoscopic cholecystectomy .
