目的：分析乳腺癌筛查在早期乳腺癌诊断中的应用，了解农村适龄妇女乳腺癌筛查的知信行(KAP)现状，旨在探讨适合农村妇女乳腺癌筛查的服务模式。方法2013年1月~2014年12月采用整群抽样的方式选取常熟市辛庄地区农村妇女为筛查对象，通过社区-医院联动的乳腺筛查及乳腺癌预防KAP问卷调查的方式分析乳腺癌筛查及KAP现状，并对比分析乳腺癌初筛者与曾接受筛查者相关情况。结果本次共筛查2098名农村妇女，共检出乳腺癌82例，检出率为3.91%，曾接受筛查人群的检出率为2.23%，明显低于初次筛查人群4.43%(P<0.05)；乳腺癌筛查的敏感性、特异性、准确性分别为91.8%、96.3%和94.3%；曾接受筛查者的乳腺癌知识、信念、行为得分均明显高于初次筛查者(P<0.05)。结论社区卫生服务机构-医院联动的筛查模式，可有效提高农村妇女乳腺癌的早诊早治率，同时应进一步加强农村妇女乳腺癌防治知识的宣传教育。
목적：분석유선암사사재조기유선암진단중적응용，료해농촌괄령부녀유선암사사적지신행(KAP)현상，지재탐토괄합농촌부녀유선암사사적복무모식。방법2013년1월~2014년12월채용정군추양적방식선취상숙시신장지구농촌부녀위사사대상，통과사구-의원련동적유선사사급유선암예방KAP문권조사적방식분석유선암사사급KAP현상，병대비분석유선암초사자여증접수사사자상관정황。결과본차공사사2098명농촌부녀，공검출유선암82례，검출솔위3.91%，증접수사사인군적검출솔위2.23%，명현저우초차사사인군4.43%(P<0.05)；유선암사사적민감성、특이성、준학성분별위91.8%、96.3%화94.3%；증접수사사자적유선암지식、신념、행위득분균명현고우초차사사자(P<0.05)。결론사구위생복무궤구-의원련동적사사모식，가유효제고농촌부녀유선암적조진조치솔，동시응진일보가강농촌부녀유선암방치지식적선전교육。
Objective To investigate application of breast cancer screening in early diagnosis of breast cancer, under-stand the status of knowledge, attitude and practice (KAP) of age women in rural areas, aimed to explore suitable ser-vice mode of breast cancer screening in rural areas. Methods With the method of cluster sampling, rural women in Xinzhuang District of Changshu City were selected as screening objects. During January 2013 to December 2014, the breast cancer screening and status of KAP were analyzed through breast screening based on the linkage of community-hospital and KAP questionnaire, and the relevant information between preliminary screening population and multiple screening population was comparatively analyzed. Results Of 2098 women in rural areas, 82 cases of breast cancer were detected, the detection rate was 3.91%. The detection rates in preliminary screening group and multiple screening group were 2.23%, 4.43%, with statistically significant difference (P<0.05). The sensitivity, specificity and accuracy of breast cancer screening was 91.8%, 96.3% and 94.3% respectively. The scores of knowledge, attitude and practice in preliminary screening group were significantly higher than those in multiple screening group (P < 0.05). Conclusion Screening model based on community health service center-hospital, can effectively improve the rates of early diagnosis and early treatment of breast cancer in age women in rural areas, at the same time the propaganda and education of prevention knowledge to breast cancer should be strengthened.
