目的:探讨扩张性心肌病患者血清尿酸(SUA)水平与功能性二尖瓣反流(FMR)的相关性,为临床诊治提供依据。方法选取2012年10月~2014年10月在本院就诊并明确诊断为扩展性心肌病的62例患者作为研究对象,入院后行SUA检测、血浆B型脑钠肽(BNP)检测和心脏超声检测,测量其左室射血分数(LVEF),检测二尖瓣反流口面积(ERO),确定二尖瓣反流程度,并检测心脏收缩期二尖瓣幕状区面积(MVA)。比较不同NYHA分级患者的SUA、BNP、LVEF和MVA以及不同程度FMR患者血清中的SUA水平,分析SUA水平与MVA的相关性。结果 NYHA评级越高,其SUA、BNP水平也越高,LVEF值越低,MVA则越大(P<0.05)。重度FMR患者的SUA水平显著高于中轻度FMR患者,差异有统计学意义(P<0.05)。相关性分析显示,扩张性心肌病患者的SUA水平与MVA呈正相关(r=0.32,P<0.01)。结论扩张性心肌病患者的SUA水平与FMR具有相关性。

목적:탐토확장성심기병환자혈청뇨산(SUA)수평여공능성이첨판반류(FMR)적상관성,위림상진치제공의거。방법선취2012년10월~2014년10월재본원취진병명학진단위확전성심기병적62례환자작위연구대상,입원후행SUA검측、혈장B형뇌납태(BNP)검측화심장초성검측,측량기좌실사혈분수(LVEF),검측이첨판반류구면적(ERO),학정이첨판반류정도,병검측심장수축기이첨판막상구면적(MVA)。비교불동NYHA분급환자적SUA、BNP、LVEF화MVA이급불동정도FMR환자혈청중적SUA수평,분석SUA수평여MVA적상관성。결과 NYHA평급월고,기SUA、BNP수평야월고,LVEF치월저,MVA칙월대(P<0.05)。중도FMR환자적SUA수평현저고우중경도FMR환자,차이유통계학의의(P<0.05)。상관성분석현시,확장성심기병환자적SUA수평여MVA정정상관(r=0.32,P<0.01)。결론확장성심기병환자적SUA수평여FMR구유상관성。
Objective To explore the correlation of serum uric acid (SUA) level and functional mitral regurgitation (FMR) in patients with dilated cardiomyopathy aiming at providing the basis for clinical diagnosis and treatment. Methods 62 patients definitely diagnosed as dilated cardiomyopathy from October 2012 to October 2014 in our hospital selected as the research objects.After admission,the SUA,plasma B-type natriuretic peptide (BNP),cardiac ultrasonogra-phy for left ventricular ejection fraction (LVEF),ERO of mitral valve for the determination of regurgitation degrees and mitral tenting area during systole were tested.The SUA,BNP,LVEF and MVA in patients with different NYHA grading was compared respectively.The level of SUA in FMR at different degrees was also compared in order to analyze the correlation of SUA level and MVA. Results The higher the NYHA grading was,the higher the levels of SUA and BNP were,the lower the LVEF value was,and the larger the MVA was (P<0.05).The level of SUA in patients with severe FMR was greatly higher than that in patients with moderate-mild FMR,with significant difference (P<0.05).Correlation analysis showed that the level of SUA had an obviously positive correlation with MVA in patients with dilated car-diomyopathy (r=0.32,P<0.01). Conclusion The level of SUA has a correlation with FMR in patients with dilated car-diomyopathy.
