目的：分析氟代脱氧葡萄糖（18 F-FDG）在原发性胃淋巴瘤（ PGL）的摄取特点，探讨PET-CT显像对其诊断价值。方法回顾性分析53例经病理确诊为PGL患者（ PGL组）的18 F-FDG PET-CT图像及临床资料，分析病灶最大标准摄取值（ SUVmax ）与D-二聚体、乳酸脱氢酶的关系。同期收集50例胃癌患者（胃癌组）、45例健康查体者（查体组）的PET-CT显像结果，对比分析PGL的PET-CT显像特点。结果胃癌组、查体组与PGL组SUVmax比较，差异均有统计学意义（ P均＜0．05）；黏膜相关性PGL、胃癌与侵袭性PGL患者SUVmax比较差异有统计学意义（P均＜0．05）。在PGL患者中，不同病理分型、临床分期间SUVmax差异有统计学意义（P均＜0．05）；D-二聚体水平与SUVmax显示较好相关性（r＝0．831，P＜0．01）。结论临床确诊或可疑PGL患者及时行PET-CT检查，结合D-二聚体水平，有利于其鉴别诊断、临床分期及治疗方案的确定。
목적：분석불대탈양포도당（18 F-FDG）재원발성위림파류（ PGL）적섭취특점，탐토PET-CT현상대기진단개치。방법회고성분석53례경병리학진위PGL환자（ PGL조）적18 F-FDG PET-CT도상급림상자료，분석병조최대표준섭취치（ SUVmax ）여D-이취체、유산탈경매적관계。동기수집50례위암환자（위암조）、45례건강사체자（사체조）적PET-CT현상결과，대비분석PGL적PET-CT현상특점。결과위암조、사체조여PGL조SUVmax비교，차이균유통계학의의（ P균＜0．05）；점막상관성PGL、위암여침습성PGL환자SUVmax비교차이유통계학의의（P균＜0．05）。재PGL환자중，불동병리분형、림상분기간SUVmax차이유통계학의의（P균＜0．05）；D-이취체수평여SUVmax현시교호상관성（r＝0．831，P＜0．01）。결론림상학진혹가의PGL환자급시행PET-CT검사，결합D-이취체수평，유리우기감별진단、림상분기급치료방안적학정。
Objective To investigate the uptake characteristics of 18 F-FDG in primary gastric lymphoma ( PGL) , and to investigate the diagnostic value of 18F-FDG positron emission tomography-computed tomography (PET-CT) imaging. Methods We studied the clinical data and 18F-FDG PET-CT imaging of 53 cases of patient with PGL (PGL group).The relationship between the SUVmax value and the relevant clinical indexes was analyzed .PET-CT imaging results were col-lected from 50 cases of gastric cancer ( gastric cancer group ) and 45 cases of normal physical examination ( control group ) , and then the comparative analysis of PET-CT imaging features of PGL was conducted .Results The SUVmax of gastric cancer group and control group was significantly different as compared with that of PGL group (all P<0.05);and the SU-Vmax of mucosa associated gastric lymphoma group and gastric cancer group was significantly different as compared with that of the aggressive primary gastric lymphoma (all P<0.05).The SUVmax of PGL in different pathologic classification and clinical stage groups was significantly different; D-dimer level and SUVmax showed a good correlation ( r=0.831, P<0.01).Conclusion The patients clinically diagnosed or suspected with PGL should receive PET-CT imaging com-bined with relevant clinical index detection in time , which is important in the differential diagnosis , clinical stages and ther-apy of primary gastric lymphoma .
