目的：探讨基层医院应用吻合器痔上黏膜环切术（procedure for prolapse and hemorrhoids，PPH）治疗Ⅲ～Ⅳ期内痔及混合痔临床效果。方法：对2012年8月至2014年7月完成的PPH术68例资料进行分析，统计患者的平均住院时间、手术时间、术中出血等临床资料。结果：本组68例病人全部治愈，住院时间2～6 d，平均住院3．2 d，手术时间20～30 min，术中出血5～20 mL，术后5例下腹坠胀，2例排便梗阻感，2例出血，14例术后尿潴留，随访无感染、复发及残留病例。结论：基层医院行PPH技术治疗痔疮简单、易掌握，手术安全有效，损伤小，恢复快，并发症少，住院时间短，值得推广。
목적：탐토기층의원응용문합기치상점막배절술（procedure for prolapse and hemorrhoids，PPH）치료Ⅲ～Ⅳ기내치급혼합치림상효과。방법：대2012년8월지2014년7월완성적PPH술68례자료진행분석，통계환자적평균주원시간、수술시간、술중출혈등림상자료。결과：본조68례병인전부치유，주원시간2～6 d，평균주원3．2 d，수술시간20～30 min，술중출혈5～20 mL，술후5례하복추창，2례배편경조감，2례출혈，14례술후뇨저류，수방무감염、복발급잔류병례。결론：기층의원행PPH기술치료치창간단、역장악，수술안전유효，손상소，회복쾌，병발증소，주원시간단，치득추엄。
Objective:To explore the clinical effects of procedure for prolapse and hemorrhoids ( PPH) on stage Ⅲ-Ⅳ internal hemorrhoids and mixed hemorrhoids.Methods:The data of the 68 cases treated with PPH from August 2012 to July 2014 were analyzed, with the average length of stay, the surgery time and the intraoperative bleeding recorded.Results:The 68 patients were all cured;the length of stay is 2-6 days;the aver-age length of stay is three days; the surgery time is 20-30 minutes, intraoperative bleeding is 5-20ml.There are 5 cases of abdominal bulge, 2 cases of bowel obstruction, 2 cases of hemorrhage and 14 cases of postoperative urinary retention.The follow-up found no cases of infection, re-currence and residual.Conclusion:PPH is simple, easy to grasp, safe and effective.Besides, it also has the advantages of less damage, faster re-covery, fewer complications, shorter hospital stay and lower expense of treatment.
