目的:探讨两种超声弹性成像( ultrasonic elastography,UE)参数直径变化率及面积比在鉴别乳腺良恶性肿瘤中的诊断价值。方法:对144个乳腺肿块进行弹性成像检查,以病理结果为金标准,分析乳腺良恶性病变弹性参数的差异。结果:乳腺病变在弹性分级>4分者,恶性肿瘤出现率明显高于良性病变( P <0.05);肿块在弹性图与二维图的最大直径变化率和面积比参数在良、恶性组间比较差异均有统计学意义( P <0.05);比较2种参数的ROC曲线发现,肿块面积最大直径变化率在鉴别恶性方面准确性更大。结论:UE能客观评价乳腺肿块的相对弹性硬度,定量参数直径变化率和面积比有助于乳腺肿块良恶性的鉴别诊断,而面积比更为准确。

ob Objective:To investigate the diagnostic value of diameter change rate and area ratio of two kinds of quantitative parameters of ul-trasonic elastography (UE) in differentiating between benign and malignant breast tumors.Methods:144 breast tumors were examined by using UE.The difference between the elastic parameters of benign and malignant breast lesions were analyzed based on the gold standard of the pathologic results.Results:For breast lesions whose elastic scores were above 4 points, the incidence rate of malignant tumors was significantly higher than that of benign lesions ( P <0.05); the parameters of the maximal diameter change rate and area ratio of the mass in the elastography and the two di-mensional figure were different between benign and malignant groups, which was significant ( P <0.05);Two parameters of ROC curve were com-pared and it was found that the minimal diameter change rate of the area of the mass was more accurate in differentiating malignant tumors .Conclu-sion:The relatively elastic rigidity can be objectively evaluated by UE; the quantitative parameters of diameter change rate and area ratio can help with the differential diagnosis of benign and malignant tumors, in which area ratio is more accurate.
