目的:研究髂骨恶性肿瘤的特点及影像表现,提高髂骨肿瘤的影像诊断及鉴别水平。方法回顾性分析33例手术病理或活检证实的髂骨肿瘤及肿瘤样病变的CT表现,分析肿瘤的破坏方式,范围,内部结构,软组织肿块大小。结果(1)髂骨恶性肿瘤病理类型多样,骨髓来源的占大部分,大多数CT表现为溶骨性骨质破坏,盆腔内形成较大的软组织肿块,恶性程度高者易侵犯关节。(2)良性肿瘤和肿瘤样病变多数以膨胀性破坏为主,边界清楚、硬化,无软组织肿块。骨软骨瘤和骨纤维结构不良有其特征性的影像表现。结论髂骨恶性肿瘤的影像表现有其特点,经过仔细观察及综合分析,大部分可定性诊断。
目的:研究髂骨噁性腫瘤的特點及影像錶現,提高髂骨腫瘤的影像診斷及鑒彆水平。方法迴顧性分析33例手術病理或活檢證實的髂骨腫瘤及腫瘤樣病變的CT錶現,分析腫瘤的破壞方式,範圍,內部結構,軟組織腫塊大小。結果(1)髂骨噁性腫瘤病理類型多樣,骨髓來源的佔大部分,大多數CT錶現為溶骨性骨質破壞,盆腔內形成較大的軟組織腫塊,噁性程度高者易侵犯關節。(2)良性腫瘤和腫瘤樣病變多數以膨脹性破壞為主,邊界清楚、硬化,無軟組織腫塊。骨軟骨瘤和骨纖維結構不良有其特徵性的影像錶現。結論髂骨噁性腫瘤的影像錶現有其特點,經過仔細觀察及綜閤分析,大部分可定性診斷。
목적:연구가골악성종류적특점급영상표현,제고가골종류적영상진단급감별수평。방법회고성분석33례수술병리혹활검증실적가골종류급종류양병변적CT표현,분석종류적파배방식,범위,내부결구,연조직종괴대소。결과(1)가골악성종류병리류형다양,골수래원적점대부분,대다수CT표현위용골성골질파배,분강내형성교대적연조직종괴,악성정도고자역침범관절。(2)량성종류화종류양병변다수이팽창성파배위주,변계청초、경화,무연조직종괴。골연골류화골섬유결구불량유기특정성적영상표현。결론가골악성종류적영상표현유기특점,경과자세관찰급종합분석,대부분가정성진단。
Objective To study the CT feature and imaging finding of malignant iliac tumor, and improve the differential diagnosis of iliac tumor. Methods The imaging features of 33 cases of iliac tumor and tumor-like lesions, proved by pathology, were analyzed retrospectively. The mode of bone destruction, tumor margin, structure of lesion, and the size of soft tissue tumor were analyzed. Results (1) iliac malignant tumors had diverse types of pathology. Bone marrow-derived accounted for the most part, the majority of which showed osteolytic bone destruction and the formation of large soft tissue mass in the pelvis, The higher the degree of malignancy tend to invade the joints. (2) The mode of bone destruction of benign tumors and tumor-like lesions was expansion with clear boundary and hardening without significant soft tissue tumor, While osteochondroma and fibrous dysplasia had its characteristic imaging findings. Conclusion The imaging findings of iliac malignant tumors have their own characteristics, observation and comprehensive analysis will help to get the accurate diagnosis.
