目的:介绍以桡侧副动脉后支为蒂的嵌合骨皮瓣游离移植修复手指复杂缺损的临床经验和手术方法。方法采用以桡侧副动脉后支为蒂的游离嵌合骨皮瓣修复8例合并指骨缺损的手指复杂缺损;手术中以相对独立的穿支为蒂切取皮瓣和骨瓣;皮瓣面积5.0 cm×2.0 cm~7.5 cm×4.0 cm,骨瓣大小1.5 cm×0.5 cm~2.5 cm×1.0 cm。结果8例嵌合骨皮瓣均成活良好;骨瓣平均愈合时间5.4(3~6)个月;供区直接闭合,仅遗留线性瘢痕;随访12~18个月,患者满意度高。结论以桡侧副动脉后支为蒂的嵌合骨皮瓣游离移植是修复复杂手部缺损的较好方法。
目的:介紹以橈側副動脈後支為蒂的嵌閤骨皮瓣遊離移植脩複手指複雜缺損的臨床經驗和手術方法。方法採用以橈側副動脈後支為蒂的遊離嵌閤骨皮瓣脩複8例閤併指骨缺損的手指複雜缺損;手術中以相對獨立的穿支為蒂切取皮瓣和骨瓣;皮瓣麵積5.0 cm×2.0 cm~7.5 cm×4.0 cm,骨瓣大小1.5 cm×0.5 cm~2.5 cm×1.0 cm。結果8例嵌閤骨皮瓣均成活良好;骨瓣平均愈閤時間5.4(3~6)箇月;供區直接閉閤,僅遺留線性瘢痕;隨訪12~18箇月,患者滿意度高。結論以橈側副動脈後支為蒂的嵌閤骨皮瓣遊離移植是脩複複雜手部缺損的較好方法。
목적:개소이뇨측부동맥후지위체적감합골피판유리이식수복수지복잡결손적림상경험화수술방법。방법채용이뇨측부동맥후지위체적유리감합골피판수복8례합병지골결손적수지복잡결손;수술중이상대독립적천지위체절취피판화골판;피판면적5.0 cm×2.0 cm~7.5 cm×4.0 cm,골판대소1.5 cm×0.5 cm~2.5 cm×1.0 cm。결과8례감합골피판균성활량호;골판평균유합시간5.4(3~6)개월;공구직접폐합,부유류선성반흔;수방12~18개월,환자만의도고。결론이뇨측부동맥후지위체적감합골피판유리이식시수복복잡수부결손적교호방법。
Objective To introduce the clinical experience and surgical methods of transplanting free chimeric radial collateral artery perforator lfaps to repair complex bone and soft-tissue defects in the hands.Methods Eight cases of digital skin defects complicated with phalanx defects were repaired by free chimeric radial collateral artery perforator lfaps. Flaps 5.0 cm×2.0 cm to 7.5 cm×4.0 cm in area and humeral bone segments 1.5 cm×0.5 cm to 2.5 cm×1.0 cm in size were pedicled on independent perforators.Results All the 8 flaps survived well and the transplanted bone fragments united to the host phalanx 3 to 6 ( averaged 5.4 months ) months after the operation. All the donor wounds were closed with direct suture with only linear scar. A 12 to 18 months’ follow-up revealed a high satisfactory rate from the patients.Conclusions The transplantation of free chimeric radial collateral artery perforator lfap is a good option to repair complex bone and soft-tissue defects in hands.
