探讨冠心病患者外周血血小板表面细胞外基质金属蛋白酶诱导因子(EMMPRIN)表达水平和血清P-选择素水平变化与冠脉病变的关系。收集2014年3~9月在我院住院的138例冠心病(CHD)患者,其中稳定性心绞痛(SAP)52例,不稳定性心绞痛(UAP)30例,急性心肌梗死(AMI)56例;另选择40例冠脉造影结果正常者作为对照组。采用二次离心法提取血小板,流式细胞仪检测血小板表面EMMPRIN表达水平;采用酶联免疫法检测血清P-选择素水平。冠心病患者血小板表面EMMPRIN表达水平和血清P-选择素表达水平显著高于对照组;随着冠状动脉病情程度加重和病变累及支数增加,血小板表面EMMPRIN表达水平和血清P-选择素水平逐渐升高(<0.05或<0.01)。相关分析显示,冠心病患者血小板表面EMMPRIN表达水平与血清P-选择素水平呈正相关(=0.596,<0.01)。冠心病患者血小板表面EMMPRIN表达水平和血清P-选择素水平明显升高,与冠心病类型及冠脉病变程度关系密切,两者对于冠心病的发生、发展均具有重要的临床意义。

To explore the relationship between extracellular matrix metalloproteinase inducer (EMMPRIN) expressions of peripheral blood platelet surfaces and serum P-selectin changes and coronary artery lesions in the patients with coronary heart diseases (CHD). Total 138 patients with CHD admitted in our hospital from March to September of 2014 were selected, including 52 ones with stable angina pectoris (SAP), 30 ones with unstable angina pectoris (UAP) and 56 ones with acute myocardial infarction (AMI). Another 40 patients with normal angiographic results of coronary artery were selected to constitute the control group. The platelets were extracted with two-step centrifugation method; platelet surface EMMPRIN expressions were tested with flow cytometer and serum P-selectin levels were examined with enzyme-linked immunosorbent assay. Platelet surface EMMPRIN expressions and serum P-selectin expressions of the patients with CHD were obviously higher than those in the control group; with the deterioration of pathogenetic conditions and the increase of lesion-involved blood vessels, EMMPRIN expressions and serum P-selectin levels rose gradually ( < 0.05 or < 0.01). The relevant analyses showed that platelet surface EMMPRIN expressions and serum P-selectin levels of the patients with CHD were positively correlated( =0.596, <0.01). Platelet surface EMMPRIN expressions and serum P-selectin levels of the patients with coronary diseases increase significantly and are closely correlated with the types of coronary diseases and the degrees of coronary artery lesions. Both of them have important clinical significances for the occurrence and development of coronary diseases.
