目的：通过三种常用镇痛方法的观察与分析，选择最合适的镇痛方法。方法随机选取2014年择期剖宫产患者150例，ASAⅠ～Ⅱ级，随机分为Ⅰ、Ⅱ、Ⅲ三组，每组50例。术毕接一次性自控镇痛泵，Ⅰ组为硬膜外镇痛泵，Ⅱ、Ⅲ组为不同配伍的静脉镇痛泵。分别记录患者术后4、8、16、24、48 h的疼痛、镇静、睡眠评分，并同时记录有无恶心、呕吐、皮肤瘙痒、尿潴留等并发症。结果Ⅰ、Ⅲ组疼痛评分明显低于Ⅱ组，有显著差异（P＜0．05）；Ⅲ组术后排气时间明显短于Ⅰ、Ⅱ组，有显著差异（ P＜0．05）；Ⅲ组睡眠良好率优于Ⅰ、Ⅱ组。结论舒芬太尼静脉自控镇痛是剖宫产患者最为合适的镇痛方法。
목적：통과삼충상용진통방법적관찰여분석，선택최합괄적진통방법。방법수궤선취2014년택기부궁산환자150례，ASAⅠ～Ⅱ급，수궤분위Ⅰ、Ⅱ、Ⅲ삼조，매조50례。술필접일차성자공진통빙，Ⅰ조위경막외진통빙，Ⅱ、Ⅲ조위불동배오적정맥진통빙。분별기록환자술후4、8、16、24、48 h적동통、진정、수면평분，병동시기록유무악심、구토、피부소양、뇨저류등병발증。결과Ⅰ、Ⅲ조동통평분명현저우Ⅱ조，유현저차이（P＜0．05）；Ⅲ조술후배기시간명현단우Ⅰ、Ⅱ조，유현저차이（ P＜0．05）；Ⅲ조수면량호솔우우Ⅰ、Ⅱ조。결론서분태니정맥자공진통시부궁산환자최위합괄적진통방법。
Objective To select the most suitableone through the observation and analysis of three general analgesic methods.Methods 150cases of the scheduled caesarean section in 2014 which were at ASA I-II level were randomly selectedand divided into Groups I, II, and III with 50 casesin each group.Once thedisposableself-controlled analgesic pump was used after the operation, the epidural analgesic pump was applied in GroupI whilethe intravenous oneswith dif-ferent compatibility were applied in GroupsII and III.Then the scores of pain, sedation, and sleep of patients were recor-ded in 4 hours, 8 hours, 16 hours, 24 hours, and 48 hoursrespectively after the operation.And the complications such as nausea, vomiting, skin itching, and urinary retention were also recorded simultaneously.Results The scoresof pain in Groups I and III were significantly lower than that in Group II( P<0.05 ).The exhaust duration in Group III was sig-nificantly shorter than those in Groups I and II (P<0.05).The rate of good sleep in Group III was higher than those in-Groups I and II.Conclusions The self-controlledintravenous analgesia with sufentanilis the most suitable method for the patients suffering from the caesarean section.
