目的:探究动机性访谈在糖尿病患者自我效能和自我管理中的应用价值。方法:选取本院2012年11月-2013年2月在内分泌科住院治疗的100例糖尿病患者,随机数字表法分为观察组和对照组,每组50例。两组患者在研究期间均接受糖尿病的基础药物治疗,对照组患者给予常规护理干预和健康教育,观察组给予综合护理干预(常规护理以及包括动机性访谈在内的健康教育)。比较6个月后患者的自我效能与自我管理量表评分,血糖控制状况和生活质量。结果:干预6个月后两组患者的自我效能和自我管理评分较干预前均有提高,观察组的平均评分高于对照组(P<0.05)。6个月后两组的空腹血糖和糖化血红蛋白较之前均有明显下降,观察组的平均空腹血糖低于对照组(P<0.05),糖化血红蛋白的差异无统计学意义(P>0.05)。观察组各项生活质量评分较之前均有明显提高,对照组仅在治疗成果上较干预前有显著提高,观察组在各个维度上的评分均明显高于对照组(P<0.05)。结论:动机性访谈指导的健康教育对糖尿病患者的自我效能和自我管理有明显的改善作用,有助于控制血糖,提高患者的生活质量。

목적:탐구동궤성방담재당뇨병환자자아효능화자아관리중적응용개치。방법:선취본원2012년11월-2013년2월재내분비과주원치료적100례당뇨병환자,수궤수자표법분위관찰조화대조조,매조50례。량조환자재연구기간균접수당뇨병적기출약물치료,대조조환자급여상규호리간예화건강교육,관찰조급여종합호리간예(상규호리이급포괄동궤성방담재내적건강교육)。비교6개월후환자적자아효능여자아관리량표평분,혈당공제상황화생활질량。결과:간예6개월후량조환자적자아효능화자아관리평분교간예전균유제고,관찰조적평균평분고우대조조(P<0.05)。6개월후량조적공복혈당화당화혈홍단백교지전균유명현하강,관찰조적평균공복혈당저우대조조(P<0.05),당화혈홍단백적차이무통계학의의(P>0.05)。관찰조각항생활질량평분교지전균유명현제고,대조조부재치료성과상교간예전유현저제고,관찰조재각개유도상적평분균명현고우대조조(P<0.05)。결론:동궤성방담지도적건강교육대당뇨병환자적자아효능화자아관리유명현적개선작용,유조우공제혈당,제고환자적생활질량。
Objective:To explore the influence of motivational interviewing on self-efficacy and self-management in patients with diabetes.Method:100 cases with diabetes in our hospital from November 2012 to February 2013 were selected and randomly divided into observation group and control group with 50 cases in each group.All patients were treated with basic drug therapy,in the control group patients were treated with routine care intervention and health education while the observation group plus comprehensive nursing interventions which included health education based on motivational interviews.Self-management and self-efficacy scores,glycemic control and quality of life after 6 months were compared.Result:Self-efficacy and self-management of the two groups were significantly improved,with an average score of the observation group higher than that of the control group (P<0.05).Fasting plasma glucose and glycated hemoglobin in the observation group and the control group were significantly decreased,the mean fasting plasma glucose of the observation group was lower (P<0.05),the difference in glycated hemoglobin between the two groups was not statistically significant (P>0.05).The quality of life of the observation group was significantly improved,and the control group only advanced in treatment outcome,The scores of the observation group were higher than that of the control group in all dimensions(P<0.05).Conclusion:Motivational interviewing which guides health education has significant improvement for diabetes’ self-efficacy and self-management,it helps control blood sugar and improve the patients’ quality of life.
