目的：观察依帕司他联合坦索罗辛治疗糖尿病神经源性膀胱（ DNB）的疗效。方法选取DNB患者60例，随机分为两组。对照组30例，给予常规膀胱训练方法治疗，治疗组30例，在常规基础治疗基础上，再给予依帕司他联合坦索罗辛治疗。记录两组治疗前、治疗4周后尿流动力学、膀胱内残余尿量和临床疗效。结果治疗后，治疗组总有效率73.3%，对照组总有效率46.7%。治疗组总有效率高于对照组，差异有统计学意义（ P <0.05）。与对照组相比，治疗组治疗后残余尿明显减少，充盈末逼尿肌压力、膀胱压力下降，最大膀胱容量，最大尿流率明显改善，两者比较差异有统计学意义（ P <0.05）。结论依帕司他联合坦索罗辛治疗DNB近期疗效确切，DNB患者生活质量明显提高，值得临床推广。
목적：관찰의파사타연합탄색라신치료당뇨병신경원성방광（ DNB）적료효。방법선취DNB환자60례，수궤분위량조。대조조30례，급여상규방광훈련방법치료，치료조30례，재상규기출치료기출상，재급여의파사타연합탄색라신치료。기록량조치료전、치료4주후뇨류동역학、방광내잔여뇨량화림상료효。결과치료후，치료조총유효솔73.3%，대조조총유효솔46.7%。치료조총유효솔고우대조조，차이유통계학의의（ P <0.05）。여대조조상비，치료조치료후잔여뇨명현감소，충영말핍뇨기압력、방광압력하강，최대방광용량，최대뇨류솔명현개선，량자비교차이유통계학의의（ P <0.05）。결론의파사타연합탄색라신치료DNB근기료효학절，DNB환자생활질량명현제고，치득림상추엄。
Objective To observe the effects of epalrestat combined with tamsulosin in treatment of diabetic neurogenic bladder. Methods 60 patients with type 2 diabetic neurogenic bladder were randomly divided into two groups:control group(n=30),receiving conventional blad-der training method only;the treatment group(n=30),receiving conventional bladder training method combined with epalrestat and tamsulosin. Urodynamic studies,bladder residual urine volume,the total effective rate were detected before and after 4 weeks treatment. Results After treat-ment,the total effective rate of control group was 46. 7%,the total effective rate of treatment group was 73. 3%,the comparative difference was statistically significant( P <0. 05). After treatment,bladder residual urine volume,the end of the filling detrusor pressure,bladder pressure of the treatment group were improved significantly compared with controls,maximum bladder capacity,maximum urinary flow rate of treatment group were improved significantly compared with controls,the comparative difference was statistically significant( P <0. 05). Conclusion The cura-tive effect of epalrestat combined with tamsulosin in treatment of diabetic neurogenic bladder can obviously improve the quality of life in patients with diabetic neurogenic bladder,which can be a clinical promotion in the future.
