目的:观察弥漫性大B细胞淋巴瘤( DLBCL)组织中O6-甲基鸟嘌呤-DNA甲基转移酶( MGMT)和人MutS同系物1( hMLH1)表达变化,并探讨其与DLBCL临床病理参数和化疗疗效的关系。方法采用免疫组织化学方法检测100例初治DLBCL患者肿瘤组织中的MGMT和hMLH1,分析两者表达的相关性及其与DLBC临床病理参数、化疗疗效的关系。结果本组患者肿瘤组织MGMT表达阳性42例,阴性58例,hMLH1表达阳性54例,阴性46例。 DLBCL患者肿瘤组织中MGMT与hMLH1表达无显著相关性。 MGMT表达与DLBCL临床分期、IPI评分、病理亚型有关(P均<0.05),与患者性别、年龄、血清LDH、全身症状、原发部位无关;hMLH1表达与DLBCL患者IPI评分、病理亚型有关( P均<0.05),与临床分期、性别、年龄、血清LDH、全身症状、原发部位无关。 MGMT阳性者CR率和化疗总有效率分别为28.6%、52.4%,均低于 MGMT阴性者的55.2%和75.9%( P均<0.05)。 hM-LH1阳性者CR率和化疗总有效率分别为75.9%,均高于hMLH1阴性者的28.3%和54.4%(P均<0.05)。结论DLBCL 组织中MGMT 和hMLH1表达率分别为42%和54%,MGMT 表达与临床分期、IPI 评分、病理亚型有关,hMLH1表达与IPI 评分、病理亚型有关,MGMT 表达阳性和hMLH1表达表达阴性者化疗效果较好。

목적:관찰미만성대B세포림파류( DLBCL)조직중O6-갑기조표령-DNA갑기전이매( MGMT)화인MutS동계물1( hMLH1)표체변화,병탐토기여DLBCL림상병리삼수화화료료효적관계。방법채용면역조직화학방법검측100례초치DLBCL환자종류조직중적MGMT화hMLH1,분석량자표체적상관성급기여DLBC림상병리삼수、화료료효적관계。결과본조환자종류조직MGMT표체양성42례,음성58례,hMLH1표체양성54례,음성46례。 DLBCL환자종류조직중MGMT여hMLH1표체무현저상관성。 MGMT표체여DLBCL림상분기、IPI평분、병리아형유관(P균<0.05),여환자성별、년령、혈청LDH、전신증상、원발부위무관;hMLH1표체여DLBCL환자IPI평분、병리아형유관( P균<0.05),여림상분기、성별、년령、혈청LDH、전신증상、원발부위무관。 MGMT양성자CR솔화화료총유효솔분별위28.6%、52.4%,균저우 MGMT음성자적55.2%화75.9%( P균<0.05)。 hM-LH1양성자CR솔화화료총유효솔분별위75.9%,균고우hMLH1음성자적28.3%화54.4%(P균<0.05)。결론DLBCL 조직중MGMT 화hMLH1표체솔분별위42%화54%,MGMT 표체여림상분기、IPI 평분、병리아형유관,hMLH1표체여IPI 평분、병리아형유관,MGMT 표체양성화hMLH1표체표체음성자화료효과교호。
Objective To observe the expression changes of O6-methylguanine DNA methyltransferase ( MGMT) and human MutL homolog 1 (hMLH1) in the diffuse large B-cell lymphoma (DLBCL) tissues and to investigate their correla-tions with DLBCL clinicopathological features and chemotherapeutic effect.Methods Immunohistochemisty was employed to determine the expression of MGMT and hMLH1 in the tumor tissues of 100 cases of newly diagnosed patients with DL-BCL, and their correlations with DLBCL clinical parameters and chemotherapeutic effect were analyzed.Results In the tumor tissues, the positive expression of MGMT and hMLH1 was found in 42 and 54 cases, and the negative expression of MGMT and hMLH1 was found in 58 and 46 cases, respectively.In the tumor tissues of DLBCL patients, MGMT was not correlated with and hMLH1.The expression of MGMT was significantly related with the clinical stage, international prog-nostic index (IPI) score and different immunohistochemical subgroups of DLBCL (all P<0.05), and was not related with sex, age, serum LDH, general symptoms and primary sites.The expression of hMLH1 was significantly related with IPI score and immunohistochemical subgroups of DLBCL (all P<0.05);and was note related with clinical stage, sex, age, serum LDH, general symptoms and primary sites.The complete remission (CR) rate (28.6%) and total effective rate of chemotherapy (52.4%) in the positive MGMT group were lower than those of the negative group (55.2%, 75.9%) (all P<0.05).The CR rate (55.6%) and total effective rate of chemotherapy (75.9%) in the positive hMLH1 group were lower than those of the negative group (28.3%, 54.4%) (all P<0.05).Conclusions The expression rates of MGMT and hMLH1 are 42%and 54%, respectively in DLBCL tissues.The expression of MGMT is significantly related with the clinical stage, IPI score and different immunohistochemical subgroups of DLBCL;and the expression of hMLH1 is signifi-cantly related with IPI score and different immunohistochemical subgroups of DLBCL.The chemotherapeutic effect of pa-tients with MGMT negative and hMLH1 positive expression is better than that of patients with the opposite expression.
