目的 观察鼠类肉瘤病毒癌基因(KRAS)和表皮生长因子受体(EGFR)在非小细胞肺癌(NSCLC)原发病灶和转移淋巴结中的表达,探讨KRAS和EGFR在NSCLC淋巴结转移中的作用.方法 收集经手术切除且病理证实的186例NSCLC患者,记录其性别、年龄、肿瘤性质、浸润程度、淋巴结转移情况及TNM分期等各项临床相关资料.将肿瘤标本行免疫组织化学染色,并提取肿瘤细胞DNA行聚合酶链反应(PCR)扩增和测序,观察EGFR和KRAS在NSCLC原发病灶和转移淋巴结中的表达及变异.结果 (1)EGFR蛋白在NSCLC组织中表达阳性率为34.41%(64例),KRAS在NSCLC组织中表达阳性率为36.56%(68例).EGFR和KRAS阳性表达与淋巴结转移明显相关(P<0.05).(2)原发病灶与淋巴结转移病灶间KRAS基因突变率差异有统计学意义(P<0.05),EGFR突变率差异无统计学意义(P>0.05).结论 EGFR和KRAS在NSCLC组织中高表达,且其表达与变异与NSCLC淋巴结转移密切相关.

Objective To investigate epidermal growth factor receptor (EGFR) and kirsten rat sarcoma viral oncogene (KRAS) expression in non-small cell lung cancer (NSCLC) and lymphatic metastases,and to explore the roles of EGFR and KRAS in NSCLC lymphatic metastases.Methods By collecting surgically resected and pathologically confirmed cases of NSCLC,get 186 cases of complete clinical data,extraction of tumor cell DNA,polymerase chain reaction (PCR) amplification and sequencing.To observe EGFR and KRAS expression in NSCLC and lymphatic metastasis,and to analyse with SPSS 14.0 statistical software.Results Positive rate of EGFR gene expression in NSCLC was 34.41% (64 cases).EGFR gene expression was positively correlated with the differentiation and lymphaden metastases.Positive rate of KRAS expression in gastric cancer was 36.56% (68 cases).KRAS expression was positively correlated with the lymphaden metastases and clinical stages.The mutation rate of KRAS had significant difference in NSCLC and lymph node metastasis,but the mutation rate of EGFR had no significant difference.Conclusion EGFR and KRAS were high expression in NSCLC tissue,and it may give some guideline for the clinical diagnosis,illness evaluation,treatment and prognosis of gastric cancer.
