目的:观察参附桃红承气汤对多器官功能障碍综合征(MODS)模型大鼠肝肾功能及组织病理的影响。方法将50只健康Wistar大鼠随机分成空白组,模型组,参附桃红承气汤低、中、高剂量组,每组10只。参照文献方法造模,自造模前3天至处死前,空白组和模型组给予生理盐水1mL灌胃,1天2次参附桃红承气汤组分别给予2、4、8g/mL生药灌胃,1天2次,观察各组大鼠造模前后生命体征。造模24h后处死大鼠取血,检测血清尿素氮(BUN)、肌酐(Cr)、总胆红素(TB)、谷丙转氨酶(ALT)、谷草转氨酶(AST),摘取肝、肾组织行病理学检查。结果模型组大鼠血清BUN、Cr、TB、ALT、AST 水平明显高于空白组[(24.26±7.1)mmol/L、(80.02±26.27)μmol/L、(4.51±1.29)μmol/L、(259.90±199.81)U/L、(345.80±209.95)U/L 比(6.82±0.62)mmol/L、(45.68±6.69)μmol/L、(0.42±0.14)μmol/L、(55.33±16.43)U/L、(108.44±15.89)U/L,P均<0.05];与模型组比较,参附桃红承气汤高剂量组BUN、Cr、TB、ALT、AST水平显著降低[(12.53±3.80)mmol/L、(56.51±14.63)μmol/L、(2.41±0.89)μmol/L、(67.00±36.27)U/L、(135.78±27.03)U/L 比(24.26±7.1)mmol/L、(80.02±26.27)μmol/L、(4.51±1.29)μmol/L、(259.90±199.81)U/L、(345.80±209.95)U/L,P均<0.05]。光镜下,参附桃红承气汤高剂量组肝肾组织较模型组炎性浸润有明显改善,出血、水肿及坏死明显减轻。结论高剂量参附桃红承气汤能降低MODS模型大鼠血清BUN、Cr、TB、ALT、AST水平,对脂多糖(LPS)介导的MODS模型大鼠肝肾组织病理有改善作用。

목적:관찰삼부도홍승기탕대다기관공능장애종합정(MODS)모형대서간신공능급조직병리적영향。방법장50지건강Wistar대서수궤분성공백조,모형조,삼부도홍승기탕저、중、고제량조,매조10지。삼조문헌방법조모,자조모전3천지처사전,공백조화모형조급여생리염수1mL관위,1천2차삼부도홍승기탕조분별급여2、4、8g/mL생약관위,1천2차,관찰각조대서조모전후생명체정。조모24h후처사대서취혈,검측혈청뇨소담(BUN)、기항(Cr)、총담홍소(TB)、곡병전안매(ALT)、곡초전안매(AST),적취간、신조직행병이학검사。결과모형조대서혈청BUN、Cr、TB、ALT、AST 수평명현고우공백조[(24.26±7.1)mmol/L、(80.02±26.27)μmol/L、(4.51±1.29)μmol/L、(259.90±199.81)U/L、(345.80±209.95)U/L 비(6.82±0.62)mmol/L、(45.68±6.69)μmol/L、(0.42±0.14)μmol/L、(55.33±16.43)U/L、(108.44±15.89)U/L,P균<0.05];여모형조비교,삼부도홍승기탕고제량조BUN、Cr、TB、ALT、AST수평현저강저[(12.53±3.80)mmol/L、(56.51±14.63)μmol/L、(2.41±0.89)μmol/L、(67.00±36.27)U/L、(135.78±27.03)U/L 비(24.26±7.1)mmol/L、(80.02±26.27)μmol/L、(4.51±1.29)μmol/L、(259.90±199.81)U/L、(345.80±209.95)U/L,P균<0.05]。광경하,삼부도홍승기탕고제량조간신조직교모형조염성침윤유명현개선,출혈、수종급배사명현감경。결론고제량삼부도홍승기탕능강저MODS모형대서혈청BUN、Cr、TB、ALT、AST수평,대지다당(LPS)개도적MODS모형대서간신조직병리유개선작용。
Objective To investigate the influence of Shenfu Taohong Chengqi Decoction on hepatic and renal function and histopathology of rats with multiple organ dysfunction syndrome(MODS). Methods Fifty healthy Wis-tar rats were randomly divided into blank group, model group, and low-, medium- and high-dose Shenfu Taohong Chengqi Decoction groups, with 10 rats in each group. MODS model was established according to the literature methods. Shenfu Taohong Chengqi Decoction groups were intragastrically given 2, 4, and 8g/mL crude drug from 3d before to 1d after modeling. Other 2 groups were given 1 mL normal saline twice a day for the same period. Vital signs of rats were observed before and after modeling. All rats were sacrificed 24h later after modeling and their blood samples were taken to determine serum urea nitrogen (BUN), creatinine (Cr), total bilirubin (TB), alanine aminotransferase (ALT), and aspertate aminotransferase (AST) and tissue samples from their liver and kidney were taken for histopathological examination. Results The levels of BUN, Cr, TB, ALT, and AST in model group were significantly higher than those in blank group (24.26 ±7.1mmol/L, 80.02 ±26.27μmol/L, 4.51 ±1.29μmol/L, 259.90 ± 199.81U/L, 345.80±209.95U/L vs 6.82±0.62mmol/L, 45.68±6.69μmol/L, 0.42±0.14μmol/L, 55.33±16.43U/L, 108.44± 15.89U/L; all P<0.05). Compared with model group, the levels of BUN, Cr, TB, ALT, and AST in high-doseShenfu Taohong Chengqi Decoction group were significantly decreased(12.53±3.80mmol/L, 56.51±14.63μmol/L, 2.41± 0.89μmol/L, 67.00±36.27U/L, 135.78±27.03U/L vs 24.26±7.1mmol/L, 80.02±26.27μmol/L, 4.51±1.29μmol/L, 259.90± 199.81U/L, 345.80±209.95U/L; all P<0.05). Under the microscope, inflammatory infiltrates in hepatic and renal tis-sues from high-dose Shenfu Taohong Chengqi Decoction group were milder than that in model group, hemorrhage, edema and necrosis were significantly reduced as well. Conclusion High dose of Shenfu Taohong Chengqi Decoc-tion can reduce serum BUN, Cr, TB, ALT, and AST levels of MODS rats, and ameliorate their lipopolysaccharide-mediated hepatic and renal histopathological changes.
