目的:探讨重症急性胰腺炎(SAP)模型大鼠不同时间点炎性因子的变化情况及桃核承气汤对其影响和作用机制。方法将90只SD大鼠随机分为空白对照组、SAP模型组、桃核承气汤组、奥曲肽组、桃核承气汤+奥曲肽组。除空白对照组外,其余各组采用20% L-精氨酸腹腔注射制备大鼠SAP模型,每组于12、24、48 h三个时间点各处死6只大鼠,采用ELISA法检测血清肿瘤坏死因子( TNF)-α、白细胞介素( IL)-6、IL-10表达水平,生化分析仪检测血清淀粉酶(AMY)、Ca2+水平,采用HE染色观察胰腺病理变化。结果与空白对照组相比,SAP模型组各时间点大鼠血清TNF-α、IL-6、IL-10水平均显著升高(P<0.05);三个治疗组与SAP模型组各检测标本组间比较有统计学差异,且胰腺出血坏死及炎症细胞浸润明显减轻(P<0.05);桃核承气汤、奥曲肽对减轻SAP大鼠炎症因子及胰腺病理变化的主效应有统计学意义(P<0.05),但两药合用不存在交互作用(P>0.05)。结论桃核承气汤能明显减少SAP大鼠TNF-α、IL-6等炎性因子的释放,提高IL-10水平,从而减轻炎症反应并对胰腺及机体起到保护作用,抑制SAP病情的发展。

목적:탐토중증급성이선염(SAP)모형대서불동시간점염성인자적변화정황급도핵승기탕대기영향화작용궤제。방법장90지SD대서수궤분위공백대조조、SAP모형조、도핵승기탕조、오곡태조、도핵승기탕+오곡태조。제공백대조조외,기여각조채용20% L-정안산복강주사제비대서SAP모형,매조우12、24、48 h삼개시간점각처사6지대서,채용ELISA법검측혈청종류배사인자( TNF)-α、백세포개소( IL)-6、IL-10표체수평,생화분석의검측혈청정분매(AMY)、Ca2+수평,채용HE염색관찰이선병리변화。결과여공백대조조상비,SAP모형조각시간점대서혈청TNF-α、IL-6、IL-10수평균현저승고(P<0.05);삼개치료조여SAP모형조각검측표본조간비교유통계학차이,차이선출혈배사급염증세포침윤명현감경(P<0.05);도핵승기탕、오곡태대감경SAP대서염증인자급이선병리변화적주효응유통계학의의(P<0.05),단량약합용불존재교호작용(P>0.05)。결론도핵승기탕능명현감소SAP대서TNF-α、IL-6등염성인자적석방,제고IL-10수평,종이감경염증반응병대이선급궤체기도보호작용,억제SAP병정적발전。
Objective To explore inflammatory factor changes on SAP rat in different time points and the effects,mechanism of Tao-hechengqidecoction.Methods 90SPFSDmaleratswererandomlydividedintoblankcontrol,SAPmodel,Taohechengqidecoctiontreated, octreotide treated,Taohechengqi decoction and octreotide combination treated groups.At the time of 12,24,48 h,each group was executed 6 rats.The serum TNF-α,IL-6,IL-10 levels were obtained by ELISA,the contents of AMY,Ca2+were measured by biochemical indicators and pathological changes were observed by HE staining.Results Serum TNF-α,IL-6,IL-10 levels were significantly increased in SAP model group compared to those of blank control(P<0.05).There were significant differences between SAP treated and SAP model group,and pan-creas hemorrhage necrosis and inflammatory cells infiltration were significantly reduced(P<0.05).There were significant differences in the effect of Taohechengqi decoction and octreotide treatment on inflammatory factors and pancreatic pathological changes(P<0.05),but with no combined interaction(P>0.05).Conclusions Taohechengqi decoction could reduce inflammatory factors,such as IL-6,TNF-αand increase anti-inflammatory factor IL-10 expressions in SAP rats.Then,it could relieve acute systemic inflammation and protect the pancreas of SAP rat,and hinder the development of SAP condition.
