目的 探讨胸腰骨折采用经伤椎椎弓根螺钉固定的治疗效果.方法 选取40例胸腰椎骨折患者作为研究对象,均采用经伤椎椎弓根螺钉固定治疗,回顾性分析其临床资料.结果 术后平均随访12个月,术中无椎间盘、血管、神经损伤,术后无椎间隙、切口感染,无脊髓、神经损伤加重.采用CT片及X线片检查,伤椎椎伤高度理想,胸腰椎曲度良好,均骨性愈合.脊髓神经功能5例为A级,3例为C级,3例为D级,29例为E级.术后1周Cobb角、伤椎椎体前缘高度与术前比较,差异有统计学意义(P＜0.05),末次随访与术后1周比较,差异未见统计学意义(P＞0.05).结论 胸腰椎骨折采用经伤椎椎弓根螺钉治疗,可显著改善预后,促使脊髓神经功能恢复,提高患者生存质量,具有非常积极的临床意义.

목적 탐토흉요골절채용경상추추궁근라정고정적치료효과.방법 선취40례흉요추골절환자작위연구대상,균채용경상추추궁근라정고정치료,회고성분석기림상자료.결과 술후평균수방12개월,술중무추간반、혈관、신경손상,술후무추간극、절구감염,무척수、신경손상가중.채용CT편급X선편검사,상추추상고도이상,흉요추곡도량호,균골성유합.척수신경공능5례위A급,3례위C급,3례위D급,29례위E급.술후1주Cobb각、상추추체전연고도여술전비교,차이유통계학의의(P＜0.05),말차수방여술후1주비교,차이미견통계학의의(P＞0.05).결론 흉요추골절채용경상추추궁근라정치료,가현저개선예후,촉사척수신경공능회복,제고환자생존질량,구유비상적겁적림상의의.
Objective To investigate the effect of pedicle screw on thoracolumbar vertebral fractures.Methods Forty cases of thoracolumbar fracture patients were selected for the study,all the patients had the vertebral pedicle screw fixation,the clinical data were retrospectively analyzed.Results The average postoperative follow-up was 12 months,no disc surgery,blood vessels,nerve injury,postoperative intervertebral space,wound infection,spinal cord,nerve damage worse occurred.Using CT slices and X-ray examination,injured vertebral vertebral height was ideal,thoracic lumbar curvature was good,all patients had bone healing.Five cases of spinal cord nerve function was grade A,3 cases of grade C,3 cases of grade D,29 cases of grade E.Cobb angle after 1 week,before vertebral vertebral height comparison with surgery,there was significant difference (P ＜ 0.05),compared with the last follow-up after 1 week,there was no significant difference(P ＞ 0.05).Conclusions Thoracolumbar vertebral fractures by pedicle screw therapy,can significantly improve the prognosis,prompt spinal cord nerve function recovery and improve the quality of life of patients,has very positive clinical significance.
