目的 利用生物电阻抗技术探讨体成分指标与稳定期慢性阻塞性肺疾病(COPD)患者生活质量之间的关系.方法 采用横断面调查的方法,对33例稳定期COPD男性患者进行一般资料、人体成分分析、心肺功能检查和圣-乔治呼吸问卷(SGRQ)的调查,对其结果进行相关性分析.结果 体质量、体脂肪量、无机盐、腰臀比与SGRQ疾病影响呈负相关.排除身高的影响,身体质量指数(BMI)、脂肪质量指数(FMI)、去脂体质量指数(FFMI)、身体水分质量指数(BFMI)、无机盐质量指数(MMI)、肌肉质量指数(SMMI)与第1秒用力呼气容积占预计值百分比(FEV1%)呈正相关,与SGRQ疾病影响呈负相关,BMI、FFMI、MMI与SGRQ总分呈负相关.排除体质量的影响,身体水分率、肌肉率与FEV1%呈负相关,与SGRQ疾病影响呈正相关.结论 BMI由FMI、FFMI、BFMI、SMMI和PMI共同影响,排除身高或体质量的影响后,体成分指标与稳定期COPD患者生活质量之间的相关性更好.

Objective To investigate the relationship between body composition indexes and quality of life in chronic obstructive pulmonary disease (COPD) patients in stable stage.Methods A simple sampling survey of 33 cases of COPD patients in stable stage was enrolled.Patients performed body composition analysis,pulmonary function tests and Saint George' s Respiratory Questionnaire (SGRQ).Analysis on the correlation between these test results was executed by Pearson correlation.Results Weight,body fat,mineral and waist-to-hip ratio(WHR) had negative correlation with disease affect of SGRQ.Eliminating the influence of the height,body mass index (BMI),fat mass index (FMI),fat free mass index(FFMI),body fluid mass index (BFMI) and mineral mass index(MMI) had positive correlation with FEV1%,and negative correlation with disease impact of SGRQ.BMI,FFMI and MMI had negative correlation with total score of SGRQ.Eliminating the influence of weight,body moisture rate and skeletal muscle rate had negative correlation with FEV1%,and positive correlation with disease impact of SGRQ.Conclusions BMI is influenced by FMI,FFMI,BFMI,SMMI and PMI.When the influence of the height or weight is eliminated,the correlation of body composition indexes with quality of life of COPD patients in stable stage is better.
