目的:探讨保留并修复后方关节囊及韧带对防止后方入路人工髋关节置换术后髋关节后脱位的作用。方法将503例采用髋关节后方入路的初次人工髋关节置换的患者分为两组:A 组298例保留并修复后方关节囊及韧带,B 组205例切除关节囊及韧带。比较两组术后髋关节后脱位发生率。结果A 组发生髋关节后脱位2例,发生率0.67%,B 组发生髋关节后脱位6例,发生率2.93%,A 组比 B 组术后髋关节后脱位发生率明显降低(P <0.05)。结论保留并修复后方关节囊及韧带用以加强人工髋关节后方软组织支撑力量,有助于恢复髋关节的软组织平衡,增加关节稳定性,对防止后方入路人工髋关节置换术后髋关节后脱位有较为重要的作用。

목적:탐토보류병수복후방관절낭급인대대방지후방입로인공관관절치환술후관관절후탈위적작용。방법장503례채용관관절후방입로적초차인공관관절치환적환자분위량조:A 조298례보류병수복후방관절낭급인대,B 조205례절제관절낭급인대。비교량조술후관관절후탈위발생솔。결과A 조발생관관절후탈위2례,발생솔0.67%,B 조발생관관절후탈위6례,발생솔2.93%,A 조비 B 조술후관관절후탈위발생솔명현강저(P <0.05)。결론보류병수복후방관절낭급인대용이가강인공관관절후방연조직지탱역량,유조우회복관관절적연조직평형,증가관절은정성,대방지후방입로인공관관절치환술후관관절후탈위유교위중요적작용。
Objective To explore the effect of retaining and repairing posterior articular capsule and ligament for pre-vention of the postoperative dislocation after hip replacement.Methods 503 cases of primary hip replacement through posterior approach were divied into two groups.298 cases were treated with hip replacement with retaining and repairing posterior articular capsule and ligament (Group A),and other 205 cases with standard hip replacement for resection of posterior capsulotendinous repair(Group B).The postoperative dislocation rate was compared between the two groups.Results 2 cases with the postoperative dislocation occurred in the Group A,but 6 occurred in the Group B.The postoperative dislocation rate of Group A was significantly less than that in Group B,which was 0.67%vs 2.93% respectively,P <0.05.Conclusions Retaining and repairing posterior articular capsule and ligament can effectively prevent early postoperative dislocation after THA.The mechanism is the retaining and repair can enhance the rear soft tissue support strength,reconstruct soft tissue balance and augment hip stability.
