目的:了解高通量透析对尿毒症患者钙磷代谢、贫血和营养状况的影响和护理体会。方法选取乐山市人民医院维持性血透患者120例,随机分为高通量透析组和低通量透析组。6个月后,分别比较两组患者前后血钙、血磷、甲状旁腺激素、血红蛋白、血白蛋白、血肌酐和尿素氮等情况。结果高通量透析组血磷和甲状旁腺激素水平明显低于低通量透析组,差异有统计学意义(P<0.01);高通量透析组观察结束时,甲状旁腺激素水平明显低于观察开始时,差异有统计学意义(P<0.05),血红蛋白水平明显高于观察开始时,差异有统计学意义(P<0.05);低通量透析组观察结束时,甲状旁腺激素水平明显高于观察开始时,差异有统计学意义(P<0.05),血钙水平明显低于观察开始时,差异有统计学意义(P<0.05)。结论高通量透析可以改善血透患者的钙磷代谢和贫血。

목적:료해고통량투석대뇨독증환자개린대사、빈혈화영양상황적영향화호리체회。방법선취악산시인민의원유지성혈투환자120례,수궤분위고통량투석조화저통량투석조。6개월후,분별비교량조환자전후혈개、혈린、갑상방선격소、혈홍단백、혈백단백、혈기항화뇨소담등정황。결과고통량투석조혈린화갑상방선격소수평명현저우저통량투석조,차이유통계학의의(P<0.01);고통량투석조관찰결속시,갑상방선격소수평명현저우관찰개시시,차이유통계학의의(P<0.05),혈홍단백수평명현고우관찰개시시,차이유통계학의의(P<0.05);저통량투석조관찰결속시,갑상방선격소수평명현고우관찰개시시,차이유통계학의의(P<0.05),혈개수평명현저우관찰개시시,차이유통계학의의(P<0.05)。결론고통량투석가이개선혈투환자적개린대사화빈혈。
Objective To analyse the effects of high-flux hemodialysis in calcium phosphorus metabolic,anemia and nu-tritional status of patients with maintenance hemodialysis dialysis and nursing care understand. Methods 120 cases of MHD pa-tients in Leshan People’s Hospital were selected and divided into two groups,which one was high-flux hemodialysis,and the other one was low-flux hemodialysis. After 6 months,serum calcium,serum phosphorus,PTH,hemoglobin,serum albumin,serum creati-nine and urea nitrogen of the patients were detected. Results The level of serum phosphorus and PTH of high-flux hemodialysis patients group reduced more obviously comparison with low-flux hemodialysis patients group(P<0. 01). After 6 months,the level of PTH of high- flux hemodialysis patients group was reduced more obviously and the level of hemoglobin was heightened ( P<0. 05). After 6 months,the level of PTH of low-flux hemodialysis patients group was heightened more obviously and the level of serum calcium was reduced(P<0. 05). Conclusion Calcium phosphorus metabolism and anemia of maintenan hemodialysis dial-ysis patients were improved with high-flux dialysis.
