目的:探讨骨科手术切口感染的病原菌特点及耐药性,为控制骨科手术切口感染提供指导。方法回顾性分析4459例骨科手术患者161例切口感染情况,对分泌物进行细菌培养及耐药性分析。结果所有切口感染161例,感染率为3.61%。共检测出致病原菌168株,其中革兰氏阴性菌97株占57.74%,主要为大肠埃希菌、肺炎克雷伯杆菌、铜绿假单胞菌;革兰氏阳性菌69株占41.07%,主要为金黄色葡萄球菌;凝固酶阴性葡萄球菌;真菌2株占1.19%。药敏试验发现革兰氏阴性菌对庆大霉素、克林霉素等的耐药率高,对头孢哌酮/舒巴坦、亚胺培南敏感性强。革兰氏阳性菌对青霉素、庆大霉素耐药率高,对万古霉素、亚胺培南敏感性强。结论骨科手术切口感染病原菌各异,加强临床病原菌及耐药性检测,为预防切口感染提供指导。

목적:탐토골과수술절구감염적병원균특점급내약성,위공제골과수술절구감염제공지도。방법회고성분석4459례골과수술환자161례절구감염정황,대분비물진행세균배양급내약성분석。결과소유절구감염161례,감염솔위3.61%。공검측출치병원균168주,기중혁란씨음성균97주점57.74%,주요위대장애희균、폐염극뢰백간균、동록가단포균;혁란씨양성균69주점41.07%,주요위금황색포도구균;응고매음성포도구균;진균2주점1.19%。약민시험발현혁란씨음성균대경대매소、극림매소등적내약솔고,대두포고동/서파탄、아알배남민감성강。혁란씨양성균대청매소、경대매소내약솔고,대만고매소、아알배남민감성강。결론골과수술절구감염병원균각이,가강림상병원균급내약성검측,위예방절구감염제공지도。
Objective To investigate antimicrobial andcharacteristics of pathogenic bacteria incision infection in Depart-ment of orthopedics operation. In order to provide guidance for controlling operation incision infection in Department of orthopedics. Methods Retrospective of 4459 cases of Department of orthopedics operation,In 161 cases incision infection of bacterial culture and drug resistance analysis of secretion. Results All 161 cases of incision infection,The infection rate was 3. 61%. Detected a total of 168 strains pathogenic Bacteria. The 97 strains of gram negative bacteria Accounted for 57. 74%,Mainly Escherichia coli、Klebsiella pneumoniae、Pseudomonas aeruginosa;69 strains of gram positive bacteria Accounted for 41. 07%;Mainly for Staphylo-coccus aureus、coagulase negative Staphylococcus;2 strains of fungi accounted for 1. 19%. The drug sensitivity test results of the high resistance rate of gram negative bacteria resistant to gentamicin clindamycin,To Cefoperazone Shubatan、imipenem sensitivity;High resistant rate of gram positive bacteria to penicillin gentamicin,High sensitivity to vancomycin imipenem. Conclusion Dif-ferent pathogens in the Department of orthopedics operation incision infection,Strengthen the clinical detection of pathogenic bacte-ria and drug resistance to provide guidance for prevention of incisioninfection.
