目的：探索肺炎型肺癌的临床特征、诊断要点，减少误诊。方法回顾性分析23例经病理证实的肺炎型肺癌患者的临床资料，包括其症状体征、影像学检查、实验室检查等资料，总结其临床特征。结果肺炎型肺癌患者以咳嗽、胸痛、呼吸困难为主要症状，肺部感染症状体征不明显。实验室检测提示白细胞、中性粒细胞比例与C反应蛋白轻度升高，大多数患者血清肿瘤标记物未见明显升高。常规抗感染治疗无效。胸部X光和CT检测提示斑片状浸润或大片模糊阴影，抗感染治疗前后CT检查无明显变化。结论肺炎型肺癌的临床表现缺乏肺部感染的一般特征，临床需积极开展病理学检查明确诊断，减少误诊。

Objective To investigate and summary the clinical characteristics and diagnostic key points of pneumonic type lung carcinoma. Methods A respective analysis the clinical summary of 23 cases of pneumonic type lung carcinoma was conducted,including symptoms,imaging examinations,laboratory tests. Results Cough,chest pain,and dyspnea were the major symptoms of patients of pneumonic type lung carcinoma. Laboratory test suggested a mild elevation of white blood cell,percentage of neutrophils,CRP and tumor markers. Patients had poor response to regular anti-infective treatments. Chest X ray and CT examina-tion suggested patchy infiltration and shadow blur,there was no significant CT changes after anti-infective treatments. Conclusion Patients with pneumonic type lung carcinoma lack typical lung infection characteristics. Pathological examination should be per-formed to establish the diagnosis and reduce misdiagnosis.
