目的:探索64排螺旋CT检测冠状动脉(冠脉)病变与血浆髓过氧化物酶(MPO)水平的相关性。方法选择以胸痛为主要症状的患者254例,根据冠脉CT造影结果,分为正常对照(NC)组89例、稳定型心绞痛(SAP)组98例、急性冠脉综合征(ACS)组67例。所有患者均测定血糖、血脂、血压等指标。采用酶联免疫吸附法测定所有入选者血浆MPO浓度,用Gensini积分评价冠脉病变严重程度,比较组间血浆MPO浓度水平差异及血浆MPO浓度与Gensini积分的相关性。结果ACS组血浆MPO平均浓度高于SAP组,SAP组血浆MPO平均浓度高于正常对照组,差异均有显著统计学意义(P<0.01)。在冠心病患者中,血浆MPO水平随着Gensini积分增加而升高(P<0.01),MPO浓度与Gensini积分呈显著正相关(r=0.51,P<0.01)。结论64排螺旋CT冠脉检查进行Gensini积分与血浆MPO水平密切相关,两者联合评估冠脉病变严重程度有一定临床意义。

목적:탐색64배라선CT검측관상동맥(관맥)병변여혈장수과양화물매(MPO)수평적상관성。방법선택이흉통위주요증상적환자254례,근거관맥CT조영결과,분위정상대조(NC)조89례、은정형심교통(SAP)조98례、급성관맥종합정(ACS)조67례。소유환자균측정혈당、혈지、혈압등지표。채용매련면역흡부법측정소유입선자혈장MPO농도,용Gensini적분평개관맥병변엄중정도,비교조간혈장MPO농도수평차이급혈장MPO농도여Gensini적분적상관성。결과ACS조혈장MPO평균농도고우SAP조,SAP조혈장MPO평균농도고우정상대조조,차이균유현저통계학의의(P<0.01)。재관심병환자중,혈장MPO수평수착Gensini적분증가이승고(P<0.01),MPO농도여Gensini적분정현저정상관(r=0.51,P<0.01)。결론64배라선CT관맥검사진행Gensini적분여혈장MPO수평밀절상관,량자연합평고관맥병변엄중정도유일정림상의의。
Objective To investigate the correlation between 64-slice spiral computed tomography (64-slice CT) and concentration of myeloperoxidas (MPO) in detection of coronary artery lesions. Methods The patients with chest pain as main symptom were chosen (n=254), and divided, according to the outcomes of coronary CT angiography, into normal group (n=89), stable angina pectoris group (SAP group, n=98) and acute coronary syndrome group (ACS group, n=67). The indexes of plasma glucose, blood fat and blood pressure were detected. The concentration of plasma MPO was detected by using enzyme-linked immunosorbent assay (ELISA) and severity of coronary artery lesions was reviewed by using Gensini integrals. The difference in MPO concentration, and correlation between MPO and Gensini integrals were analyzed. Results The mean concentration of plasma MPO was higher in ACS group than that in SAP group, and was higher in SAP group than that in normal group (P<0.01). In CHD patients, the concentration of plasma MPO increased as Gensini integrals ascended (P<0.01), and MPO was positively correlated to Gensini integrals (r=0.51, P<0.01). Conclusion Gensini integrals and MPO is closely related to each other during 64-slice CT, which have some clinical value in reviewing severity of coronary artery lesions.
