目的:分析冠状动脉粥样硬化性心脏病(冠心病)患者非高密度脂蛋白胆固醇(non-HDL-C)/高密度脂蛋白胆固醇(HDL-C)(即动脉硬化指数,AI)、低密度脂蛋白胆固醇(LDL-C)/HDL-C、血浆致动脉硬化指数(AIP)和血脂综合指数(LCI)与冠状动脉病变程度的相关性。方法选择第二炮兵总医院心血管内科2008年1月至2011年12月因胸痛、心前区不适住院行冠状动脉(冠脉)造影的患者148例,其中男性98例,女性50例,年龄范围40~65岁。按冠脉造影结果分为冠心病组(86例)和对照组(62例)。检测所有患者总胆固醇(TC)、三酰甘油(TG)、LDL-C、HDL-C,计算non-HDL-C、AI值、LDL-C/HDL-C比值、AIP和LCI。并采用Gensini积分法评定冠状动脉病变程度,分析血脂指标与冠脉病变程度的相关性。结果与对照组比较,冠心病组TC、LDL-C、non-HDL-C、AI、LDL-C/HDL-C值显著增加,有显著统计学差异(P均<0.01);HDL-C降低,有统计学差异(P<0.05)。Spearman等级相关分析显示,non-HDL-C、AI、LCI和LDL-C/HDL-C比值与冠状动脉病变程度呈正相关(r=0.247,r=0.314,r=0.178,r=0.278,P均<0.05)HDL-C水平与Gensini积分呈负相关,相关系数-0.231(P<0.05)。结论 AI在对照组和冠心病组存在明显差异,且与冠状动脉病变严重程度呈显著正相关。

목적:분석관상동맥죽양경화성심장병(관심병)환자비고밀도지단백담고순(non-HDL-C)/고밀도지단백담고순(HDL-C)(즉동맥경화지수,AI)、저밀도지단백담고순(LDL-C)/HDL-C、혈장치동맥경화지수(AIP)화혈지종합지수(LCI)여관상동맥병변정도적상관성。방법선택제이포병총의원심혈관내과2008년1월지2011년12월인흉통、심전구불괄주원행관상동맥(관맥)조영적환자148례,기중남성98례,녀성50례,년령범위40~65세。안관맥조영결과분위관심병조(86례)화대조조(62례)。검측소유환자총담고순(TC)、삼선감유(TG)、LDL-C、HDL-C,계산non-HDL-C、AI치、LDL-C/HDL-C비치、AIP화LCI。병채용Gensini적분법평정관상동맥병변정도,분석혈지지표여관맥병변정도적상관성。결과여대조조비교,관심병조TC、LDL-C、non-HDL-C、AI、LDL-C/HDL-C치현저증가,유현저통계학차이(P균<0.01);HDL-C강저,유통계학차이(P<0.05)。Spearman등급상관분석현시,non-HDL-C、AI、LCI화LDL-C/HDL-C비치여관상동맥병변정도정정상관(r=0.247,r=0.314,r=0.178,r=0.278,P균<0.05)HDL-C수평여Gensini적분정부상관,상관계수-0.231(P<0.05)。결론 AI재대조조화관심병조존재명현차이,차여관상동맥병변엄중정도정현저정상관。
Objective To analyze the correlation among non-high-density lipoprotein-cholesterol (non-HDL-C)/high-density lipoprotein-cholesterol (HDL-C) [i.e. atherosclerosis index (AI)], low-density lipoprotein-cholesterol (LDL-C)/HDL-C, atherogenic index of plasma (AIP), lipoprotein combine index (LCI) and coronary lesion severity in patients with coronary heart disease (CHD). Methods The patients hospitalized due to precordial discomfort and undergone coronary angiography (n=148, male 98, female 50 and aged from 40 to 65) were chosen from Jan. 2008 to Dec. 2011. All patients were divided into CHD group (n=86) and control group (n=62) according to the outcomes of coronary angiography. The levels of total cholesterol (TC), triglyceride (TG), LDL-C and HDL-C were detected, and non-HDL-C, AI, LDL-C/HDL-C, AIP and LCI were calculated in all patients. The coronary lesion severity was assessed by using Gensini method, and correlation between serum lipid parameters and coronary lesion severity was analyzed. Results Compared with control group, TC, LDL-C, non-HDL-C, AI and LDL-C/HDL-C increased significantly (all P<0.01), and HDL-C decreased (P<0.05) in CHD group. Spearman analysis showed that non-HDL-C, AI, LCI and LDL-C/HDL-C were positively correlated to coronary lesion severity (r=0.247, r=0.314, r=0.178, r=0.278, all P<0.05), and HDL-C was negatively correlated to Gensini integral (r=-0.231, P<0.05). Conclusion Atherogenic index shows significant difference between control group and CHD group, and positively correlates to coronary lesion severity.
