目的:探讨 CT 增强扫描联合三维重建技术在分化型甲状腺癌术前 T 分期的价值,提高 CT 对甲状腺癌术前评估的意义。方法搜集经手术病理证实为分化型甲状腺癌且术前行 CT 增强扫描的患者80例,分析术前 CT 增强及多种重建图像特点判断术前 T 分期,并与术后病理的 T 分期相对比,用卡方检验计算出 CT 增强扫描联合多种三维重建技术对甲状腺癌术前 T 分期的准确性、灵敏度和特异度。结果 CT 诊断 T1期的准确性、灵敏度和特异度分别为91.3%、83.3%和94.6%;T2期的准确性、灵敏度和特异度分别为90.0%、85.7%和88.5%;T3期的准确性、灵敏度和特异度分别为96.3%、90.9%和98.3%;T4a 期的准确性、灵敏度和特异度均为100%。结论 CT 增强扫描联合三维重建技术有助于甲状腺癌术前 T 分期的评估,尤其是 T3和 T4a 期。

목적:탐토 CT 증강소묘연합삼유중건기술재분화형갑상선암술전 T 분기적개치,제고 CT 대갑상선암술전평고적의의。방법수집경수술병리증실위분화형갑상선암차술전행 CT 증강소묘적환자80례,분석술전 CT 증강급다충중건도상특점판단술전 T 분기,병여술후병리적 T 분기상대비,용잡방검험계산출 CT 증강소묘연합다충삼유중건기술대갑상선암술전 T 분기적준학성、령민도화특이도。결과 CT 진단 T1기적준학성、령민도화특이도분별위91.3%、83.3%화94.6%;T2기적준학성、령민도화특이도분별위90.0%、85.7%화88.5%;T3기적준학성、령민도화특이도분별위96.3%、90.9%화98.3%;T4a 기적준학성、령민도화특이도균위100%。결론 CT 증강소묘연합삼유중건기술유조우갑상선암술전 T 분기적평고,우기시 T3화 T4a 기。
Objective To explore the evaluation of CT enhancement scan with three dimensional reconstruction on the T stage of differentiated thyroid carcinoma and to improve the value of CT before surgery .Methods Images of CT enhance‐ment scan and kinds of reconstruction technology of 80 cases proved differentiated thyroid carcinoma by pathology were an‐alyzed .The accuracy ,sensitivity and specificity of T stage diagnosed by CT were analyzed by Chi‐square test ,and com‐pared with T stage confirmed by pathology .Results The accuracy ,sensitivity and specificity of T1 stage diagnosed by CT were 91 .3% ,83 .3% and 94 .6% ,respectively ,those of T2 stage were 90 .0% ,85 .7% and 88 .5% respectively ,those of T3 stage were 96 .3% ,90 .9% and 98 .3% respectively ,and all those of T4a stage were 100% .Conclusion CT enhance‐ment scan with three dimensional reconstruction technology has good performance on the T stage ,especially the T3 and T4a stage .
