目的:对伴轻度认知损害( MCI)的晚发型抑郁症( LOD)患者脑白质胆碱能通路的改变进行对照性研究,并分析其与患者认知功能各领域受损的关系。方法:对伴MCI的LOD患者(研究组,n=25)磁共振成像的胆碱能通路高信号评分( CHIPS),同时评定蒙特利尔认知评估量表( MoCA),分析CHIPS评分与MoCA各认知领域的关系;并与不伴MCI的LOD患者(对照组,n=25)进行对照。结果:①研究组MoCA总分、视空间与执行功能、延迟记忆、注意及工作记忆得分,均显著低于对照组,差异有统计学意义(t=2.091~2.398,P=0.041~0.028);②研究组CHIPS显著高于对照组,差异有统计学意义(t=2.097,P=0.042);③研究组CHIPS评分与MoCA总分、视空间与执行功能、延迟记忆得分显著负相关(r=-0.434,-0.398,-0.483;P<0.05)。结论:脑白质胆碱能通路受损,在LOD患者MCI的发生中可能起了一定的作用,尤其与其视空间与执行功能、记忆及延迟记忆的损害有关。

목적:대반경도인지손해( MCI)적만발형억욱증( LOD)환자뇌백질담감능통로적개변진행대조성연구,병분석기여환자인지공능각영역수손적관계。방법:대반MCI적LOD환자(연구조,n=25)자공진성상적담감능통로고신호평분( CHIPS),동시평정몽특리이인지평고량표( MoCA),분석CHIPS평분여MoCA각인지영역적관계;병여불반MCI적LOD환자(대조조,n=25)진행대조。결과:①연구조MoCA총분、시공간여집행공능、연지기억、주의급공작기억득분,균현저저우대조조,차이유통계학의의(t=2.091~2.398,P=0.041~0.028);②연구조CHIPS현저고우대조조,차이유통계학의의(t=2.097,P=0.042);③연구조CHIPS평분여MoCA총분、시공간여집행공능、연지기억득분현저부상관(r=-0.434,-0.398,-0.483;P<0.05)。결론:뇌백질담감능통로수손,재LOD환자MCI적발생중가능기료일정적작용,우기여기시공간여집행공능、기억급연지기억적손해유관。
Objective:To investigate the features of white matter cholinergic pathway impairment in late-onset depression( LOD) with mild cognitive impairment( MCI) and its relationship with the dysfunc-tion of cognition.Methods:Twenty-five cases of LOD comorbidity MCI( study group) and 25 LOD with-out MCI ( control group ) were scanned with cholinergic pathways hyperintensities scale ( CHIPS ) using MR and completed the Montreal cognitive assessment( MoCA).The relation between CHIPS and MoCA was analyzed.Results:①Participants in study group scored lower in total MoCA,visual space and execu-tive abilities,attention and working memory,delayed memory than that in the control group( t=2.091~2.398,P<0.05).②The results of CHIPS were significant higher in LOD patients with MCI than those without(t =2.097,P=0.042).③In correlation analysis,negative relationships were showed between CHIPS and scores of total MoCA, visual space and executive abilities, delayed memory ( r =-0.434,-0.398,-0.483;P<0.05).Conclusion:The results indicate that the cholinergic pathway impairments imply the pathogeny of MIC in LOD,especially the lesions of visual space and executive abilities or de-layed memory.
