目的:探讨亚临床甲状腺功能减退对老年糖尿病患者血脂、C反应蛋白和心功能的影响。方法将合并亚临床甲状腺功能减退及非亚临床甲状腺功能减退的老年糖尿病患者,作为观察组和对照组,每组各240例,观察和比较两组患者血清促甲状腺素(TSH)、甘油三酯(TG)、总胆固醇(TC)、C反应蛋白(CRP)、脑钠肽(BNP)、左室射血分数(LVEF)水平。结果观察组与对照组TSH分别为(6.5±2.1)mU/L、(1.8±1.0)mU/L,TG分别为(1.8±0.5)mmol/L、(1.4±0.4)mmol/L,TC 分别为(5.8±0.7)mmol/L、(4.4±0.4)mmol/L,观察组均明显高于对照组,P<0.05;CRP分别为(4.8±1.4)mg/L、(3.1±1.3)mg/L,BNP分别为(94.2±11.3)pg/mL、(72.1±10.7)pg/mL,观察组均明显高于对照组;LVEF分别为(56.4±8.5)%、(61.5±9.0)%,观察组水平显著降低,P<0.05。结论亚临床甲状腺功能减退能够导致老年糖尿病患者血脂代谢异常,CRP水平升高,左心室收缩功能降低。

목적:탐토아림상갑상선공능감퇴대노년당뇨병환자혈지、C반응단백화심공능적영향。방법장합병아림상갑상선공능감퇴급비아림상갑상선공능감퇴적노년당뇨병환자,작위관찰조화대조조,매조각240례,관찰화비교량조환자혈청촉갑상선소(TSH)、감유삼지(TG)、총담고순(TC)、C반응단백(CRP)、뇌납태(BNP)、좌실사혈분수(LVEF)수평。결과관찰조여대조조TSH분별위(6.5±2.1)mU/L、(1.8±1.0)mU/L,TG분별위(1.8±0.5)mmol/L、(1.4±0.4)mmol/L,TC 분별위(5.8±0.7)mmol/L、(4.4±0.4)mmol/L,관찰조균명현고우대조조,P<0.05;CRP분별위(4.8±1.4)mg/L、(3.1±1.3)mg/L,BNP분별위(94.2±11.3)pg/mL、(72.1±10.7)pg/mL,관찰조균명현고우대조조;LVEF분별위(56.4±8.5)%、(61.5±9.0)%,관찰조수평현저강저,P<0.05。결론아림상갑상선공능감퇴능구도치노년당뇨병환자혈지대사이상,CRP수평승고,좌심실수축공능강저。
Objective To study the influence of subclinical hypothyroidism (SCH) on the levels of serum lipid, C-reactive protein and cardiac function for elderly diabetic patients .Methods Elderly diabetic patients with combined SCH and without SCH were regarded as the subjects of the observation group and the control group , with 240 patients in each group .The levels of serum thyroid stimulating hor-mone ( TSH) , triglyceride ( TG) , total cholesterol ( TC) , C-reaction protein ( CRP) , brain natriureic peptide ( BNP) , left ventricular e-jection fraction ( LVEF) in the two groups were observed and compared .Results Through comparing with the control group , the levels of TSH(6.5 ±2.1 vs 1.8 ±1.0)mU/L,TG(1.8 ±0.5 vs 1.4 ±0.4)mmol/L, TC (5.8 ±0.7 vs 4.4 ±0.4)mmol/L were significantly in-creased in the observation group (P<0.05);through comparing with the control group , the levels of CRP(4.8 ±1.4 vs 3.1 ±1.3)mg/L, BNP (94.2 ±11.3 vs 72.1 ±10.7)pg/mL were obviously increased and the level of LVEF (56.4 ±8.5 vs 61.5 ±9.0)%was significantly reduced in the observation group (P<0.05).Conclusion The subclinical hypothyroidism can lead to the abnormal lipid metabolism , ele-vate the level of CRP , and reduce the left ventricular systolic function .
