目的：观察Orr式Roux－en－Y吻合术和功能性空肠间置代胃术（ FJI）对于全胃切除的胃癌患者术后远期并发症及生活质量的影响。方法将96例胃癌全胃切除术后患者随机分为Orr组和FJI组，每组各48例。 Orr组消化道重建方式为Orr式Roux－en－Y吻合术，而FJI组为郝氏重建设计的FJI术式，1年及2年后对两组患者生活质量、营养状况、术后远期并发症进行比较研究。结果术后1年，除体重变化两组差异不明显外，其他指标FJI组均有优势（ P＜0．05）；术后2年，FJI组生活质量、营养状况及术后远期并发症的发生情况均明显优于Orr组（P＜0．05）。结论 FJI术式对于提高全胃切除术后患者的生活质量、营养状况，降低术后远期并发症均有明显优势，值得推广。
목적：관찰Orr식Roux－en－Y문합술화공능성공장간치대위술（ FJI）대우전위절제적위암환자술후원기병발증급생활질량적영향。방법장96례위암전위절제술후환자수궤분위Orr조화FJI조，매조각48례。 Orr조소화도중건방식위Orr식Roux－en－Y문합술，이FJI조위학씨중건설계적FJI술식，1년급2년후대량조환자생활질량、영양상황、술후원기병발증진행비교연구。결과술후1년，제체중변화량조차이불명현외，기타지표FJI조균유우세（ P＜0．05）；술후2년，FJI조생활질량、영양상황급술후원기병발증적발생정황균명현우우Orr조（P＜0．05）。결론 FJI술식대우제고전위절제술후환자적생활질량、영양상황，강저술후원기병발증균유명현우세，치득추엄。
Objective To observe effect of functional jejunal interposition (FJI)and Roux-en-Y anastomosis(Orr) on quality of life and nutritional status in total gastrectomy patients .Methods Choosing 96 total gastrectomy patients , and randomly dividing them into two groups, with 48 patients in each group.Orr group was given Roux -en-Y anastomosis while FJI group received FJI by Hao′s reconstruc-tion design.1 and 2 year later, the patients′quality of life, complications and nutritional status were compared .Results The weight change was little between 2 groups 1 year later , and the other indicators in FJI group have significant advantages .2 years later , the quality of life, nutritional status and long -term complications in FJI group were significantly better than Orr Group .Conclusion FJI for impro-ving the total gastrectomy patients′quality of life , nutritional status and reducing long -term postoperative complications have obvious ad-vantages, so it is worth promoting.
