目的：观察合并地中海贫血（地贫）对非霍奇金淋巴瘤（ NHL）化疗相关性贫血的影响及益髓生血颗粒的保护作用。方法对38例NHL合并地贫患者与38例NHL未合并地贫患者进行回顾性分析，观察第1疗程CHOP方案时化疗过程中的血红蛋白（ Hb）水平并进行比较。对38例NHL合并地贫者在接受第2疗程CHOP化疗时随机分为2组，治疗组19例同时应用益髓生血颗粒；对照组单纯接受化疗，观察Hb水平变化。结果合并地贫的NHL患者接受CHOP化疗后平均Hb下降为（8．6±5．2） g／L，与未合并地贫组（5．8±4．4） g／L相比差异有统计学意义（ P＝0．013）。合并地贫的NHL患者在接受第2疗程CHOP方案化疗时，若治疗组Hb平均下降（2．2±2．3）g／L，对照组为平均下降（4．5±4．1）g／L，差异有统计学意义（P＝0．0398）。结论南方地区NHL合并地贫患者并不少见，在接受化疗时贫血更为严重，化疗时使用益髓生血颗粒有一定的保护作用。
목적：관찰합병지중해빈혈（지빈）대비곽기금림파류（ NHL）화료상관성빈혈적영향급익수생혈과립적보호작용。방법대38례NHL합병지빈환자여38례NHL미합병지빈환자진행회고성분석，관찰제1료정CHOP방안시화료과정중적혈홍단백（ Hb）수평병진행비교。대38례NHL합병지빈자재접수제2료정CHOP화료시수궤분위2조，치료조19례동시응용익수생혈과립；대조조단순접수화료，관찰Hb수평변화。결과합병지빈적NHL환자접수CHOP화료후평균Hb하강위（8．6±5．2） g／L，여미합병지빈조（5．8±4．4） g／L상비차이유통계학의의（ P＝0．013）。합병지빈적NHL환자재접수제2료정CHOP방안화료시，약치료조Hb평균하강（2．2±2．3）g／L，대조조위평균하강（4．5±4．1）g／L，차이유통계학의의（P＝0．0398）。결론남방지구NHL합병지빈환자병불소견，재접수화료시빈혈경위엄중，화료시사용익수생혈과립유일정적보호작용。
Objective To observe the effect of thalassemia carrier on the chemotherapy induced anemia in patients with non -Hodking lymphoma(NHL)andprotectionroleofYisuishengxuegranule.Methods 38patientswithNHLcomplicatedwiththalssemiawerestudied. 38 patients with NHL not complicated with thalssemia were selected .Hemoglobin levels among the first course of CHOP chemotherapy were detected .Those patients with thalassemia were further classified into groups in the second course of chemotherapy : for treatement group , Yisuishengxue granules were used together with chemotherapy ,and for observe group ,merely chemotherapy was adapted .Also hemoglobin levels were detected during chemotherapy .Results Through comparing with patients without thalssemia ,hemoglobin decreased level was significantly higher in patients with thalssemia (8.6 ±5.2)g/L vs (5.8 ±4.4)g/L,with significant statistical significance (P=0.013). During the second course of chemotherapy in patients with NHL complicated with thalassemia ,decreased hemoglobin level in patients treated with Yisuishengxue granule was significantly lower than those patients not treated with Yisuishengxue granule (2.2 ±2.3)g/L vs( 4.5 ± 4.1 ) g/L,with significant statistical meaning , P=0.0398.Conclusion Patients′concurrent NHL and thalassemia is common in Southern China.Concurrent thalassemia aggravates chemotherapy induced anemia in patients with NHL .Yisuishenxue granule may alleviate chemo-therapy induced anemia in patients with NHL complicated with thalassemia .
