目的：探讨B超引导经皮肾镜取石术联合超声负压吸附系统治疗鹿角型肾结石的疗效。方法：对120例鹿角型肾结石患者行B超引导下标准通道经皮肾镜取石术联合超声负压吸附系统治疗，观察其结石取净率及并发症。结果：120例（128侧）鹿角型肾结石取净率82.8%，部分性鹿角型结石取净率87.1%；完全性鹿角型结石取净率71.4%。大出血行输血治疗13例，肾包膜下血肿8例，肾周积液5例。术后出现全身炎症反应综合征7例，感染性休克1例。结论：B超引导经皮肾镜取石术联合超声负压吸附系统治疗鹿角型肾结石具有清石效率高，术后并发症少等优点。
목적：탐토B초인도경피신경취석술연합초성부압흡부계통치료록각형신결석적료효。방법：대120례록각형신결석환자행B초인도하표준통도경피신경취석술연합초성부압흡부계통치료，관찰기결석취정솔급병발증。결과：120례（128측）록각형신결석취정솔82.8%，부분성록각형결석취정솔87.1%；완전성록각형결석취정솔71.4%。대출혈행수혈치료13례，신포막하혈종8례，신주적액5례。술후출현전신염증반응종합정7례，감염성휴극1례。결론：B초인도경피신경취석술연합초성부압흡부계통치료록각형신결석구유청석효솔고，술후병발증소등우점。
Objective To investigate the B ultrasound-guided percutaneous nephrolithotomy(percutaneous nephrolithotomy, PCNL) with ultrasonic vacuum adsorption system in the treatment of staghorn calculi. Methods Retrospective analysis of clinical data of 120 cases of ultrasound-guided standard channel PCNL in the treatment of staghorn calculi was studied,and the stone clearance rate and complications were observed. Results One hundred and 20 cases of PCNL were used in the treatment of staghorn calculi, a total of 128 sides. complete staghorn calculi 35 sides and partical staghorn calculi 93 sides. The total stone clearance rate was 82.81%, stone clearance rate of partical staghorn calculi was 87.10%; stone clearance rate of complete stag?horn calculi was 71.43%. Blood transfusion was given in 13 cases, subcapsular hematoma in 8 cases and perire?nal effusion in 5 cases. Postoperative systemic inflammatory response syndrome was responded in 7 cases and septic shock in 1 case. Conclusion B ultrasound-guided percutaneous nephrolithotomy with ultrasonic vacu?um adsorption system can be used in the treatment of staghorn calculi with a high stone-free rate with few com?plications.
