目的:探讨颧牙槽嵴处植入微螺钉种植体在支抗推上磨牙向远中移位中的效果。方法:52例轻中度安氏Ⅱ类畸形患者随机分为颧牙槽嵴处植入组(A组)和上颌第一磨牙颊侧根部与第二前磨牙之间植入组(B组)。记录两组患者种植成功率情况,分析两组患者治疗前和矫正后头颅定位侧位片。结果:两组患者治疗前后头颅定位侧位片测量指标差值SNA、 SNB、 ANB、 U6-Y、 U1-Y和SN-OP差异均无统计学意义(P>0.05),两组患者治疗前后U6-X、 U1-X和SN-MP差异均具有统计学意义(P<0.05); A组患者初次种植成功率100.0%,高于B组的88.9%,但差异无统计学意义(P>0.05), A组推磨牙所需时间短于B组, A组磨牙移动速度和磨牙移动距离均大于B组,差异均具有统计学意义(P>0.05)。结论:颧牙槽嵴处植入微螺钉种植体效果更佳,更有利于垂直向压入磨牙及对抗切牙唇倾,可加快支抗推上磨牙向远中移位速度,缩短恢复时间。

Objective: Discussion at the alveolar zygomatic implants implanted micro screw anchorage onto the distal molar displacement in effect. Methods: 52 cases of mild to moderate Class Ⅱ (occlusal) malformations were randomly divided between the alveolar crest zygomatic implant group (A) and the buccal roots of the maxillary first molar and second premolar implant group (B group). Two groups of patients planting success rate recorded, analyzed two groups of patients before and after correction of cephalometric radiographs. Results: Two groups of patients before and after treatment cephalometric radiographs measure of the difference between the SNA, SNB, ANB, U6-Y, U1-Y and SN-OP was no significant difference(P>0.05), two groups of patients before and after treatment U6- X, U1-X and SN-MP differences were statistically significant (P<0.05). A group of patients with the initial planting suc-cess rate of 100.0 percent, 88.9 percent higher than group B, but the difference was not statistically sig-nificant (P>0.05), A group is shorter than the time required to push the molars group B, A set of molars and molars move away from the moving speed were greater than group B, the differences were statisti-cally significant(P>0.05). Conclusion: Alveolar crest zygomatic implants implanted micro screws better, more conducive to push vertical molar and incisor inclination confrontation can accelerate anchorage onto molars to the shift speed, shorten recovery time.
