目的:探讨微移技术(inching)在腕管综合征(CTS)中的应用价值。方法:对符合临床诊断标准的41例CTS患者和32例正常对照者,均进行微移技术、常规法及比较法的神经传导检测,比较三者对诊断CTS的敏感性及特异性。结果:微移技术检测显示CTS组在腕下2cm至腕下3cm节段的正中神经潜伏期差值显著长于对照组,并且检测的敏感性最高(92.7%),明显高于常规法及比较法(除外正中/尺神经环指-腕末端感觉潜伏期差),阳性预测值及阴性预测值均大于90%,具有较高的诊断价值。结论:微移技术检测是一个敏感、安全、简便、实用的方法,对CTS的诊断、定位及治疗方式的选择均有重要意义。

목적:탐토미이기술(inching)재완관종합정(CTS)중적응용개치。방법:대부합림상진단표준적41례CTS환자화32례정상대조자,균진행미이기술、상규법급비교법적신경전도검측,비교삼자대진단CTS적민감성급특이성。결과:미이기술검측현시CTS조재완하2cm지완하3cm절단적정중신경잠복기차치현저장우대조조,병차검측적민감성최고(92.7%),명현고우상규법급비교법(제외정중/척신경배지-완말단감각잠복기차),양성예측치급음성예측치균대우90%,구유교고적진단개치。결론:미이기술검측시일개민감、안전、간편、실용적방법,대CTS적진단、정위급치료방식적선택균유중요의의。
Objective:To investigate the applied value of Inching technology in carpal tunnel syn‐drome (CTS) .Methods :Consecutive 41 patients with CTS and 32 normal controls were enrolled to under‐go inching technology ,routine and comparistire tests ,and the sensitivity and specificity of the 3 devices in the diagnosis of CTS .Results:The median nerve action potential latency difference from two centimeter to three centimeter below the wrist in the CTS group was prolonged more than in controls through inching technology testing .In addition to the distal sensory latency difference between median and ulnar nerves from the ring finger to the wrist ,the sensitivity detected by inching technology was the highest (92 .7% ) in the diagnosis of CTS ,significantly higher than that by the routine and comparitive tests ,and the posi‐tive predictive value and negative predictive value exceeded 90% ,having rather high diagnostic value .Con‐clusion:Inching technology testing is a sensitive ,safe ,simple and practical device ,and is significat in the diagnosis in positioning of CTS and the choice of treatment of CTS .
