目的:观察改良本体感觉神经肌肉促进技术(PNF)对脑卒中偏瘫合并踝阵挛患者下肢功能的治疗作用。方法:将脑卒中偏瘫合并踝阵挛患者40例随机分为观察组和对照组各20例。对照组患者采用常规 PN F技术治疗,观察组患者应用改良PN F技术,即主要利用对踝关节牵伸、背伸和外翻肌群的主动力量的训练,加强患者的主动踝背伸和外翻,加强胫前肌、腓骨长短肌力量,从而有效改变踝阵挛。结果:训练20d后,2组患者综合痉挛量表(CSS)、下肢Fugl‐Meyer评定(FMA)、Berg平衡功能评分(BBS)及10m最大步行速度(10m MWS)均较治疗前有明显好转(P<0.05),观察组各项评分改善幅度更高于对照组(P<0.05)。结论:改良下肢PNF技术能更有效地提高偏瘫恢复前期踝阵挛的控制,促进下肢运动分离,提高步行能力及速度,适用于国内脑卒中患者踝阵挛的痉挛治疗。
目的:觀察改良本體感覺神經肌肉促進技術(PNF)對腦卒中偏癱閤併踝陣攣患者下肢功能的治療作用。方法:將腦卒中偏癱閤併踝陣攣患者40例隨機分為觀察組和對照組各20例。對照組患者採用常規 PN F技術治療,觀察組患者應用改良PN F技術,即主要利用對踝關節牽伸、揹伸和外翻肌群的主動力量的訓練,加彊患者的主動踝揹伸和外翻,加彊脛前肌、腓骨長短肌力量,從而有效改變踝陣攣。結果:訓練20d後,2組患者綜閤痙攣量錶(CSS)、下肢Fugl‐Meyer評定(FMA)、Berg平衡功能評分(BBS)及10m最大步行速度(10m MWS)均較治療前有明顯好轉(P<0.05),觀察組各項評分改善幅度更高于對照組(P<0.05)。結論:改良下肢PNF技術能更有效地提高偏癱恢複前期踝陣攣的控製,促進下肢運動分離,提高步行能力及速度,適用于國內腦卒中患者踝陣攣的痙攣治療。
목적:관찰개량본체감각신경기육촉진기술(PNF)대뇌졸중편탄합병과진련환자하지공능적치료작용。방법:장뇌졸중편탄합병과진련환자40례수궤분위관찰조화대조조각20례。대조조환자채용상규 PN F기술치료,관찰조환자응용개량PN F기술,즉주요이용대과관절견신、배신화외번기군적주동역량적훈련,가강환자적주동과배신화외번,가강경전기、비골장단기역량,종이유효개변과진련。결과:훈련20d후,2조환자종합경련량표(CSS)、하지Fugl‐Meyer평정(FMA)、Berg평형공능평분(BBS)급10m최대보행속도(10m MWS)균교치료전유명현호전(P<0.05),관찰조각항평분개선폭도경고우대조조(P<0.05)。결론:개량하지PNF기술능경유효지제고편탄회복전기과진련적공제,촉진하지운동분리,제고보행능력급속도,괄용우국내뇌졸중환자과진련적경련치료。
Objective:To investigate the therapeutic effectiveness of improved PNF technique on hemiplegic patient with ankle clonus .Method:Forty patients with ankle clonus were randomly divided into observation group and con‐trol group (n=20 each) .The observation group received improved PNF technique ,which has ability to effectively improve the spasm of the hemiplegic patient with ankle clonus by strengthening the strength of tibialis anterior mus‐cle .The control group received routine PNF technique .Results:Twenty days after treatment ,CSS ,FMA of lower limbs ,Berg at baseline and the speed of walk in ten metres in both two groups were significantly higher than those before treatment (P<0 .05) ,more significantly in the observation group than in the control group (P<0 .05) .Con‐clusion:The improved PNF technique can significantly improve the ankle clonus of previous rehabilitation ,promote separation of movement on lower limbs ,and heighten the speed and ability of walk .The improved PNF technique is applied to treat the hemiplegic patients with ankle clonus .
