目的：探讨居家护理模式对出院患者自理能力及生活质量的影响。方法采用自身前后对照的实验设计，对262例出院患者实施居家护理，分为出院前准备和出院后延伸服务两个阶段，内容包括出院前评估与方案制订、健康指导、家庭访视、电话随访、网络咨询。评估工具包括改良Barthel指数、SF‐36健康状况问卷。结果干预后患者的自理能力评分和SF‐36评分显著高于干预前（P＜0．05），差异均有统计学意义。结论居家护理模式能够改善出院患者的生活质量，提高患者的自理能力，实现了对出院患者全面、全程地护理，保证了出院患者在出院后能够得到专业支持和帮助，是一种先进有效的延伸护理服务模式。

Objective To evaluate he effectiveness on self‐care ability and quality of life by home nursing model for discharged patients .Method Self controlled experimental design was used .262 cases of discharged patients was given home nursing .The services was divided into preparation plan before discharge and extension services after dis‐charge from hospital ,including pre discharge evaluation and planning ,health guidance ,home visits ,telephone fol‐low‐up ,network consulting .Assessment tools were the modified Barthel index and SF‐36 .Result The patients'SF‐36 score and Barthel Index scores after the intervention were significantly higher than that before intervention . There was significant difference(P<0 .05) .Conclusion The home nursing model can improve the quality of life and self‐care ability among discharged patients .This model provides the patients with professional support and help when they were discharged from the hospital .It is an advanced and effective extension nursing service model .
