目的:探讨DPP-4抑制剂和缬沙坦对糖尿病肾病的保护机制。方法(1)链脲佐菌素制备大鼠糖尿病肾病模型并分别给予DPP-4抑制剂维格列汀和缬沙坦灌胃;(2)治疗12 w后观察各组血糖、24 h尿蛋白定量,免疫组化法检测各组肾皮质中结缔组织生长因子( CTGF)水平。结果 DN组肾组织着色广泛浓集,以肾小球表现最突出,肾小管和间质亦有所增强,维格列汀和缬沙坦治疗组,着色范围相似,但程度明显轻于DN组( P<0.01),但两组间无显著性差异;维格列汀和缬沙坦联合治疗组与两组间有差异, P<0.01,且CTGF水平明显低于DM组。结论 DPP-4抑制剂维格列汀和缬沙坦在一定程度上可以减轻糖尿病大鼠肾脏的损伤,且两组联用作用更强。

목적:탐토DPP-4억제제화힐사탄대당뇨병신병적보호궤제。방법(1)련뇨좌균소제비대서당뇨병신병모형병분별급여DPP-4억제제유격렬정화힐사탄관위;(2)치료12 w후관찰각조혈당、24 h뇨단백정량,면역조화법검측각조신피질중결체조직생장인자( CTGF)수평。결과 DN조신조직착색엄범농집,이신소구표현최돌출,신소관화간질역유소증강,유격렬정화힐사탄치료조,착색범위상사,단정도명현경우DN조( P<0.01),단량조간무현저성차이;유격렬정화힐사탄연합치료조여량조간유차이, P<0.01,차CTGF수평명현저우DM조。결론 DPP-4억제제유격렬정화힐사탄재일정정도상가이감경당뇨병대서신장적손상,차량조련용작용경강。
Objective To investigate the protection mechanism of DPP-4 inhibitor and valsartan on diabetic nephropathy. Meth-ods Streptozotocin was used to establish mouse models of diabetic nephropathy on which intragastric administration was conducted with DPP-4 inhibitor, vildagliptin, and valsartan respectively. After 12-week treatment, the blood sugar and 24 h urine protein quantita-tive of each group were tested, and the CTGF in renal cortex was detected with immunohistochemistry. Results Staining in kidney is-sue of DN group was extensive and concentrated, especially in glomerular, and that in renal tubule and interstitial also enhanced. In vildagliptin treatment group and valsartan treatment group, the coloring range was similar, but the degree was significantly less than that of DN group (P<0. 01), with no significant difference between the two groups. There was difference between the combined treat-ment group and the two groups mentioned above ( P<0. 01 ) , and the CTGF level of the former group was lower than that of the DM group. Conclusion DPP-4 inhibitor, vildagliptin, and valsartan can reduce the damage to the kidney of mice with diabetes to a cer-tain extent, and the combined treatment is more effective.
