目的:探讨脑卒中患者 H cy水平的表达变化与神经功能缺损程度的相关性。方法选取我院收治的351例脑卒中患者,根据病人入院时神经功能缺损评分(NIHSS)分为轻度、中度、重度神经功能缺损,观察不同程度缺损患者 Hcy表达水平变化。同时选取320例非脑卒中健康者为对照组,比较脑卒中组与对照组血清 Hcy、血浆纤维蛋白原(FIB)、甘油三酯(TG)、血糖(GLU)、总胆固醇(TC)以及高低密度脂蛋白(HDL‐C、LDL‐C)的差异;并将上述指标与NIHSS进行Logistic回归分析。结果随着神经功能缺损程度的增加,患者HCy表达水平亦呈递增式升高,差异有统计学意义(P<0.05)。脑卒中组患者的血清 Hcy、FIB、TG、GLU、TC、LDL‐C明显高于对照组(P<0.05),HDL‐C低于对照组(P<0.05)。Logistic回归分析显示,HCy水平、TC、FIB与NIHSS呈正相关(r=0.675,P<0.05;r=0.042,P<0.05;r=0.039,P<0.05);HDL‐C、LDL‐C、GLU、TG与NIHSS无显著相关性(r=0.006,P>0.05;r=0.012,P>0.05;r=0.008,P>0.05;r=0.009,P>0.05)。结论血浆HCy 水平表达变化与脑卒中患者的神经功能缺损呈正相关。

목적:탐토뇌졸중환자 H cy수평적표체변화여신경공능결손정도적상관성。방법선취아원수치적351례뇌졸중환자,근거병인입원시신경공능결손평분(NIHSS)분위경도、중도、중도신경공능결손,관찰불동정도결손환자 Hcy표체수평변화。동시선취320례비뇌졸중건강자위대조조,비교뇌졸중조여대조조혈청 Hcy、혈장섬유단백원(FIB)、감유삼지(TG)、혈당(GLU)、총담고순(TC)이급고저밀도지단백(HDL‐C、LDL‐C)적차이;병장상술지표여NIHSS진행Logistic회귀분석。결과수착신경공능결손정도적증가,환자HCy표체수평역정체증식승고,차이유통계학의의(P<0.05)。뇌졸중조환자적혈청 Hcy、FIB、TG、GLU、TC、LDL‐C명현고우대조조(P<0.05),HDL‐C저우대조조(P<0.05)。Logistic회귀분석현시,HCy수평、TC、FIB여NIHSS정정상관(r=0.675,P<0.05;r=0.042,P<0.05;r=0.039,P<0.05);HDL‐C、LDL‐C、GLU、TG여NIHSS무현저상관성(r=0.006,P>0.05;r=0.012,P>0.05;r=0.008,P>0.05;r=0.009,P>0.05)。결론혈장HCy 수평표체변화여뇌졸중환자적신경공능결손정정상관。
Objective To investigate the correlation between the change of Hcy level and the degree of nervous functional defect in the patients with stroke.Methods 351 cases with stroke in our hospital were selected and divided into mild ,moder‐ate ,severe neurological deficit according to the neural function defect score(NIHSS) ,observing the changes of Hcy levels in different degree of defect. At the same time ,320 healthy cases were selected as control group. The serum Hcy ,plasma fibrino‐gen(FIB) ,triglyceride(TG) ,blood glucose(GLU) ,total cholesterol(TC) and high density lipoprotein(HDL‐C) ,low density lipoprotein(LDL‐C) between the stroke group and control group were compared;which were analyzed by Logistic regression a‐nalysis. Results With the increasing degree of neurological deficiency ,the expression level of HCy also increased incrementally with significant difference(P<0.05). The serum Hcy ,FIB ,TG ,GLU ,TC ,LDL‐C in stroke group were significantly higher than that in control group(P<0.05) ,and HDL‐C was obviously lower (P<0.05). Logistic regression analysis showed that HCy ,TC ,FIB levels were positively correlated with NIHSS scores (r=0.675 ,P<0.05 ;r=0.042 ,P<0.05 ;r=0.039 ,P<0.05);the serum HDL‐C ,LDL‐C ,GLU ,TG had no significant correlation with NIHSS scores (r=0.006 ,P>0.05 ;r=0.012 ,P>0.05 ;r=0.008 ,P>0.05 ;r=0.009 ,P>0.05). Conclusion Conclusion Neurological impairment is positively correlated with serum HCy levels in patients with stroke.
