目的：探讨临床药师在心房颤动患者抗凝治疗中的作用。方法：回顾性分析临床药师参与1例心房颤动患者华法林抗凝治疗的临床资料。结果：临床药师对患者进行血栓栓塞及出血风险评估，并依据评估结果制定和调整华法林抗凝方案，对患者实施药学监护，保障了患者用药安全。结论：临床药师在心房颤动患者华法林抗凝治疗中发挥了重要作用，可降低患者抗凝治疗风险，提高患者抗凝治疗成功率。
目的：探討臨床藥師在心房顫動患者抗凝治療中的作用。方法：迴顧性分析臨床藥師參與1例心房顫動患者華法林抗凝治療的臨床資料。結果：臨床藥師對患者進行血栓栓塞及齣血風險評估，併依據評估結果製定和調整華法林抗凝方案，對患者實施藥學鑑護，保障瞭患者用藥安全。結論：臨床藥師在心房顫動患者華法林抗凝治療中髮揮瞭重要作用，可降低患者抗凝治療風險，提高患者抗凝治療成功率。
목적：탐토림상약사재심방전동환자항응치료중적작용。방법：회고성분석림상약사삼여1례심방전동환자화법림항응치료적림상자료。결과：림상약사대환자진행혈전전새급출혈풍험평고，병의거평고결과제정화조정화법림항응방안，대환자실시약학감호，보장료환자용약안전。결론：림상약사재심방전동환자화법림항응치료중발휘료중요작용，가강저환자항응치료풍험，제고환자항응치료성공솔。
OBJECTIVE:To investigate the role of clinical pharmacists in the warfarin anticoagulation treatment for patients with atrial fibrillation. METHODS: The clinical data obtained by clinical pharmacists during warfarin anticoagulation treatment for one patient with atrial fibrillation were analyzed retrospectively.RESULTS: Clinical pharmacists conducted risk assessment on thromboembolism and bleeding for the patient, formulated and adjusted the warfarin anticoagulation therapy based on the assessment and provided pharmaceutical care for the patients to ensure patients'medication safety.CONCLUSIONS: Clinical pharmacists play an important role in warfarin anticoagulation treatment for patients with atrial fibrillation in that they can help reduce the risk while improving the success rate of the anticoagulation treatment.
