目的:探讨外科手术经后路有限减压、USS系统固定联合病椎骨水泥椎体重塑技术治疗有神经压迫症状的腰椎转移性肿瘤患者的可行性、方法及临床效果。方法对2009年1月至2012年3月,我科收治21例腰椎转移性肿瘤患者,采用后入路对病变节段有限减压、USS系统重建脊柱序列稳定性、联合骨水泥椎体重塑技术的治疗方法,治疗腰椎转移性肿瘤21例,其中男性14例,女性7例。所有治疗患者均存在神经系统损害和严重疼痛。结果手术时间为120~180 min,出血量为500~800 mL。21例患者中所有患者(100%)术后疼痛即刻得到缓解。随访1~3个月,17例患者术后神经功能得到持续改善,随访期内无内固定松动、断裂,病椎椎体高度维持较好,神经功能均有明显恢复,生活质量得到显著提高。结论后入路有限减压USS系统固定联合直视下病椎骨水泥椎体重塑技术是治疗有神经压迫症状的腰椎转移性肿瘤的一种有效方法。

목적:탐토외과수술경후로유한감압、USS계통고정연합병추골수니추체중소기술치료유신경압박증상적요추전이성종류환자적가행성、방법급림상효과。방법대2009년1월지2012년3월,아과수치21례요추전이성종류환자,채용후입로대병변절단유한감압、USS계통중건척주서렬은정성、연합골수니추체중소기술적치료방법,치료요추전이성종류21례,기중남성14례,녀성7례。소유치료환자균존재신경계통손해화엄중동통。결과수술시간위120~180 min,출혈량위500~800 mL。21례환자중소유환자(100%)술후동통즉각득도완해。수방1~3개월,17례환자술후신경공능득도지속개선,수방기내무내고정송동、단렬,병추추체고도유지교호,신경공능균유명현회복,생활질량득도현저제고。결론후입로유한감압USS계통고정연합직시하병추골수니추체중소기술시치료유신경압박증상적요추전이성종류적일충유효방법。
Objective To explore the techniQue and effect of Posterior limited decompression and USS system with direct vertebral remodeling techniQues in the treatment of lumbar spinal metastases. Methods We used posterior limited decompres-sion and USS system with direct vertebral remodeling techniQues as the treatment of 21 patients,of which 14 males and 7 fe-males,who had the the diagnosis of lumbar spinal metastases between January 2009 and March 2012 in our Spine Surgery De-partment. All patients had nervous system damage and severe pain. Results The operation time was 120~180 min,the blood loss was 500~800 mL. The symptom of low back pain instantly be relieved after operation in 21 patients( 100%). After 3 months,17 cases of patient′s nerve function had been sustained improvement. Implant loosening and fracture was not found,and disease vertebral height was maintaining good,all the patient′s Quality of life was improved significantly. Conclusion Treat-ment of spinal metastatic tumors using posterior limited decompression and USS system combined direct vertebral remodeling techniQues Can instantly stabilize the vertebral. This technology can prevent vertebral paraplegia because of the pathological fractures and other complications,expand the range of application of bone cement technologies,reduces the risk of cement leak-age,while the technology can decompression effectively,relieve the severe pain,improve neurological symptoms,reduce the trauma and tumor spread opportunity and improve the Quality of life of patients. Therefore we believe this technology is an ef-fective techniQue to cure lumbar spinal metastases.
