目的：探析前列腺癌根治术患者保留部分前列腺尿道及耻骨前列腺韧带（PP）对减少尿失禁的临床疗效。方法前列腺癌患者104例，根据入院先后顺序分为两组，对照组50例行耻骨后前列腺癌根治术（RRP）治疗，观察组54例在保留部分前列腺尿道及 PP 的基础上行 RRP 治疗。结果观察组术后1、3、6、12个月完全控尿率分别为51．9％（28／54）、87．0％（47／54）、92．6％（50／54）及98．1％（53／54），均显著高于对照组[22．0％（11／50）、44．0％（22／50）、76．0％（38／50）及88．0％（44／50）]，各时段的 MHU 评分和 ICIQ－SF 评分也均显著低于对照组，差异均具有统计学意义（P 均＜0．05）。结论前列腺癌根治术患者保留部分前列腺尿道及 PP，可有效提高完全控尿率，改善尿失禁等级评分及自我评分。
목적：탐석전렬선암근치술환자보류부분전렬선뇨도급치골전렬선인대（PP）대감소뇨실금적림상료효。방법전렬선암환자104례，근거입원선후순서분위량조，대조조50례행치골후전렬선암근치술（RRP）치료，관찰조54례재보류부분전렬선뇨도급 PP 적기출상행 RRP 치료。결과관찰조술후1、3、6、12개월완전공뇨솔분별위51．9％（28／54）、87．0％（47／54）、92．6％（50／54）급98．1％（53／54），균현저고우대조조[22．0％（11／50）、44．0％（22／50）、76．0％（38／50）급88．0％（44／50）]，각시단적 MHU 평분화 ICIQ－SF 평분야균현저저우대조조，차이균구유통계학의의（P 균＜0．05）。결론전렬선암근치술환자보류부분전렬선뇨도급 PP，가유효제고완전공뇨솔，개선뇨실금등급평분급자아평분。
Objective To explore the clinical efficacy of retaining part of prostatic urethra and puboprostatic ligaments (PP)of patients with prostate cancer in reducing urinary incontinence .Methods 104 patients with prostate cancer were chosen as research objects and divided into the control group with 50 cases and the observation group with 54 cases according to the order of admission.The control group were treated by radical prostatectomy (RRP),while the observation group were treated by RRP on the basis of retaining part of prostatic urethra and PP .Results Complete urinary continence rates of the observation group after 1,3,6,12 months were 51.9%(28 /54),87.0%(47 /54),92.6%(50/54),and 98.1%(53 /54),which were significantly high-er than those of the control group of 22.0%(11 /50),44.0%(22 /50),76.0%(38 /50),and 88 /0%(44/50).MHU scores of each period and ICIQ-SF scores of the observation group were significantly lower than those of the control group .The differences were statistically significant (P <0.05).Conclusion During RRP,retaining part of prostatic urethra and PP can effectively im -prove complete urinary continence rate ,improve the level of urinary incontinence scores and self score .
