目的 探讨在头颅侧位片上测量牙弓Spee曲度的方法并探究整平Spee氏曲线与所需间隙量的关系.方法 纳入108例错(牙合)畸形患者,收集治疗前、后头颅侧位片,使用液晶数位描图板描绘Spee氏曲线,并在MATLAB7.0中提取出,基于最小二乘法进行曲线拟合、计算.结果 Spee氏曲线拟合数学模型圆的方差最小;同一操作者的自身重复性好,不同操作者间的一致性高;与模型测量法结果未见统计学差异;预测整平本研究样本全牙列和后牙段Spee氏曲线单侧所需平均间隙量分别为:1.1±0.02mm;0.87±0.08 mm.结论 1.在头颅侧位片上可以使用圆作为数学模型,采用最小二乘法曲线拟合的方法测量Spee氏曲线.2.本研究的方法可以推算整平Spee氏曲线所需的间隙量.

Objective The purpose of this study was to put forward a measurement method of Spee's curve on cepholometric radiographs,and measure Spee's curve radius and depth to predict spaces needed for leveling the curve of Spee.Methods 108 orthodontic patients were enrolled in this study.Initial and final cepholometric radiographs were collected.The curve of Spee were painted in image rendering software Adobe Photoshop CS5 (Adobe Systems,Inc,USA) by using Hanwang LCD digital tracing board (HanWan Technology Co,China).In MATLAB7.0 (MathWorks,USA),the mapped curve of Spee were extracted based on the least square curve fitting methodThe best mathematical model of Spee's curve was validated.On the basis of this research,we measured the curve of Spee's radius and depth.Results (1) When fitting Spee's curve on cepholometric radiographs through the least square method,circle was the best mathematical model with minimum variance;(2) The same operator had good repeatability and different operators had high consistency in using the Spee's curve measurement method;(3)According to this method,the average spaces needed for leveling full denture Spee's curve on one side was 1.1 ± 0.02 mm,while posterior teeth Spee's curve on one side required an average of 0.87±0.08 mm.Conclusions Circle could be used as best mathematical model when fitting Spee's curve on cepholometric radiographs through the least square method.The method in this study could be used to calculate the spaces needed when leveling the curve of Spee.
