目的分析视神经管减压术与单纯激素冲击疗法在治疗视神经管骨折上的疗效。方法回顾性总结2005年8月至2014年6月经CT证实的视神经管骨折患者共51例(51只眼),其中24例行视神经管减压术,27例行大剂量激素冲洗治疗,对比两种治疗方法的疗效。结果视神经管减压术组患者术后7 d视力提高者10例,有效率42%,单纯激素冲击组患者甲强龙500 mg冲击3 d,减量至300 mg冲击3 d,再改成口服强的松片(每公斤体重0.8 mg),逐渐减量,冲击治疗开始至第7天,视力提高者9例,有效率33%,两个治疗组对比,(P =0.539>0.05)对比组之间的差异无显著统计学意义。结论视神经减压术与单纯激素冲击治疗对视神经管骨折患者的视力预后在一定时间段内疗效无明显差异。

Objective To analyze the efficacy between optic canal decompression and high-dose corticosteroid. Methods A retrospective study of 51 optic canal fracture patients who were diagnosed by CT between Aug,2005 and Jun, 2014. 24 patients were supposed to treat with optic canal decompression while the other 27 patients were treated with high-dose corticosteroid. Then we contrast the efficacy between these two methods. Results In the optic canal decompression group,7 days after surgery,10 out of 24 had improved vision and the effective rate was 42% . And in the high-dose corti-costeroid group,all patients were treated with Methyprednisolon through intravenous instillation,500mg/ d for 3 days and then 300mg/ d for 3 days. Then oral hydroprednisone was used with 0. 8mg per kilo of weight and the dosage was reduced individually. 7 days after high-dose corticosteroid,9 of 27 had improved vision and the effective rate was 33% . There is no significant difference between the two methods (P = 0. 539 > 0. 05). Conclusion The prognostic vision has no significant difference between optic canal decompression and high-dose corticosteroid in treating the optic canal fracture patients in a short period.
