目的:探讨呼吸机相关性肺炎(VAP)的病原菌分布情况以及替加环素联合舒巴坦在VAP治疗中的应用效果。方法选取2012年1月~2014年12月本院收治的120例行机械通气后并发VAP患者作为研究对象,随机分为治疗组和对照组,各60例。治疗组采用替加环素联合舒巴坦静脉滴注,对照组采用美罗培南联合舒巴坦静脉滴注。对其下呼吸道分泌物病原菌进行培养,比较两组的治疗效果及不良反应发生率。结果下呼吸道痰液共分离出178株病原菌,其中革兰氏阴性菌(以铜绿假单胞菌、鲍曼不动杆菌为主)与革兰氏性阳性菌(以金黄色葡萄球菌为主)分离率分别占74.16%与19.67%,真菌为6.18%。治疗组的总显效率为73.3%,显著高于对照组的55.5%,差异有统计学意义(P<0.05)。两组的不良反应发生率比较,差异无统计学意义(P>0.05)。结论替加环素联合舒巴坦治疗VAP效果显著,值得临床推广应用。

목적:탐토호흡궤상관성폐염(VAP)적병원균분포정황이급체가배소연합서파탄재VAP치료중적응용효과。방법선취2012년1월~2014년12월본원수치적120례행궤계통기후병발VAP환자작위연구대상,수궤분위치료조화대조조,각60례。치료조채용체가배소연합서파탄정맥적주,대조조채용미라배남연합서파탄정맥적주。대기하호흡도분비물병원균진행배양,비교량조적치료효과급불량반응발생솔。결과하호흡도담액공분리출178주병원균,기중혁란씨음성균(이동록가단포균、포만불동간균위주)여혁란씨성양성균(이금황색포도구균위주)분리솔분별점74.16%여19.67%,진균위6.18%。치료조적총현효솔위73.3%,현저고우대조조적55.5%,차이유통계학의의(P<0.05)。량조적불량반응발생솔비교,차이무통계학의의(P>0.05)。결론체가배소연합서파탄치료VAP효과현저,치득림상추엄응용。
Objective To explore pathogenic bacteria distribution of ventilator-associated pneumonia(VAP) and the ap-plication effect of tigecycline combined with sulbactam in the treatment of VAP. Methods 120 VAP patients with me-chanical ventilation from January 2012 to December 2014 in our hospital were selected and randomly divided into the treatment group and the control group,60 cases in each group.The treatment group was treated with intravenous drip of tigecycline combined with sulbactam,the control group was treated with intravenous drip of meropenem with sulbactam. Pathogenic bacteria from secretions of the lower respiratory tract was cultivated,the treatment effect and the incidence of adverse reaction in two groups was compared. Results There were 178 strains of pathogenic bacteria separated from sputum of lower respiratory tract,the separation rates of gram-negative bacterium mainly included Pseudomonas aerugi-nosa and Baumanii was 74.16%,the separation rates of gram-positive bacterium principally contained Staphylococcus aure us was19.67%,the separation rates of fungus was 6.18%.The clinical marked improvement rate in the treatment group was 73.3%,which was higher than 55.5%in the control group,with significant difference(P<0.05).The incidence of adverse reaction in the two groups was not statistically significant (P>0.05). Conclusion The effect of tigecycline com-bined with sulbactam is remarkable,it is worthy of clinical promotion and application.
