目的：比较腰椎后路椎弓根内固定手术联合不同减压方式治疗腰椎间盘突出症的临床疗效。方法将52例接受腰椎后路椎弓根内固定术的腰椎间盘突出症患者随机分为2组，A 组30例患者应用局限椎板开窗减压术，B 组22例患者应用广泛椎板切除减压术。采用 JOA 评分标准对术前术后患者的症状和体征进行评分。结果 A 组随访28例，B 组全部获得随访，随访时间3个月~3.5 a，平均1.5 a。2组患者术后症状较术前缓解明显，A 组患者术后优良率93.3%，B 组优良率90.9%，2组间差异无统计学意义（P ＞0.05）。结论腰椎固定手术采用局限椎板开窗减压术术后效果满意，手术创伤小，腰椎稳定结构破坏少，具有良好的应用价值。

Objective Comparing the clinical analysis of lumbar posterior fixation surgery with different decompression methods in treatment of lumbar disc disease. Methods The retrospective analysis was performed on the clinical data from the patients with lumbar disc disease in our hospital from 2008 to 2013. There were 52 cases patients with lumbar posterior fixation surgery. In which 30 cases of patients in group A were used fenestration decompression method. The other 22 cases patients of group B were used laminectomy decompression method. To assess the patientsˊ symptoms and signs according to JOA scores before and after operation. Results All the patients were followed - up ex-cept 2 cases of group A. The patients were followed up 3 months to 3. 5 years,1. 5 years on average. The symptoms and signs were significantly better than that before operation in both groups. The excellent rate postoperative was A 93. 3% and B 90. 9% . There were no statistical differ-ence between the two groups(P > 0. 05). Conclusion Lumbar posterior fixation surgery with fenestration decompression method achieves satis-factory clinical results. Less trauma and less destruction of the stable structures make it has excellent application value.
