目的:探讨血管细胞黏附分子1(VCAM-1)、晚期氧化蛋白产物(AOPP)在妊娠期糖尿病(GDM)中的表达水平并进行相关性研究。方法选取2010年1月至2012年12期间40例GDM患者为研究对象( GDM组),另选择正常妊娠孕妇40名为对照组(正常妊娠组)。检测受试者母血及脐血 VCAM-1、AOPP、空腹血糖( FBG)和糖化血红蛋白( HbA1c)、载脂蛋白 A (APOA)、载脂蛋白B(APOB)水平,对两组相关指标进行统计分析。结果 GDM组母血VCAM-1、AOPP、APOB、FBG和HbA1c水平均高于正常妊娠组,差异均有统计学意义(t值分别为37.64、27.19、7.98、8.79、7.53,均P<0.05),GDM组APOA水平低于正常妊娠组,差异有统计学意义(t=14.16,P<0.05);GDM组脐血VCAM-1、AOPP水平高于正常妊娠组,差异均有统计学意义(t值分别为19.37、12.56,均P<0.01);母血清VCAM-1、AOPP表达与APOB、FBG、HbA1c、脐血VCAM-1、脐血AOPP的表达均呈正相关(r值分别为0.468~0.551、0.498~0.598,均P<0.05),与APOA的表达呈负相关(r值分别为-0.459、-0.462,均P<0.05);母血清VCAM-1与AOPP表达呈正相关(r=0.572,P<0.05)。结论 GDM患者母血清及脐血中VCAM-1、AOPP明显高表达,VCAM-1和AOPP的高表达与GDM发病、进展密切相关,VCAM-1和AOPP可作为监测GDM病情及进展的指标。

목적:탐토혈관세포점부분자1(VCAM-1)、만기양화단백산물(AOPP)재임신기당뇨병(GDM)중적표체수평병진행상관성연구。방법선취2010년1월지2012년12기간40례GDM환자위연구대상( GDM조),령선택정상임신잉부40명위대조조(정상임신조)。검측수시자모혈급제혈 VCAM-1、AOPP、공복혈당( FBG)화당화혈홍단백( HbA1c)、재지단백 A (APOA)、재지단백B(APOB)수평,대량조상관지표진행통계분석。결과 GDM조모혈VCAM-1、AOPP、APOB、FBG화HbA1c수평균고우정상임신조,차이균유통계학의의(t치분별위37.64、27.19、7.98、8.79、7.53,균P<0.05),GDM조APOA수평저우정상임신조,차이유통계학의의(t=14.16,P<0.05);GDM조제혈VCAM-1、AOPP수평고우정상임신조,차이균유통계학의의(t치분별위19.37、12.56,균P<0.01);모혈청VCAM-1、AOPP표체여APOB、FBG、HbA1c、제혈VCAM-1、제혈AOPP적표체균정정상관(r치분별위0.468~0.551、0.498~0.598,균P<0.05),여APOA적표체정부상관(r치분별위-0.459、-0.462,균P<0.05);모혈청VCAM-1여AOPP표체정정상관(r=0.572,P<0.05)。결론 GDM환자모혈청급제혈중VCAM-1、AOPP명현고표체,VCAM-1화AOPP적고표체여GDM발병、진전밀절상관,VCAM-1화AOPP가작위감측GDM병정급진전적지표。
Objective To study the expression levels of vascular cell adhesion molecule 1 ( VCAM-1 ) and advanced oxidation protein products ( AOPP) in gestational diabetes mellitus and its correlation. Methods Between January 2010 and December 2012 40 patients with GDM were selected in research ( GDM group) , and another 40 cases of normal pregnancy were selected in control group ( normal pregnant group). The levels of VCAM-1,AOPP, fasting blood glucose (FBG), glycosylated hemoglobin (HbA1c), apolipoprotein A (APOA) and apolipoprotein B (APOB) in maternal blood and cord blood were detected, and the related indicators were analyzed. Results In GDM group VCAM-1, AOPP, APOB, FBG and HbA1c levels in maternal blood were significantly higher than those in normal pregnancy group (t value was 37. 64, 27. 19, 7. 98, 8. 79 and 7. 53, respectively, all P<0. 01), but APOA level was lower (t=14. 16,P<0. 05). In GDM group VCAM-1 and AOPP levels in umbilical cord blood were obviously higher than those in normal pregnancy group ( t value was 19. 37 and 12. 56, respectively, both P<0. 01). Maternal VCAM-1 level was positively correlated with serum APOB, FBG, HbA1c, and VCAM-1 and AOPP in umbilical cord blood (r value was 0. 468, 0. 529, 0. 551, 0. 512 and 0. 536, respectively, all P<0. 05), and maternal AOPP was also positively correlated with them ( r value was 0. 498, 0. 592, 0. 598, 0. 526 and 0. 532, respectively, all P<0. 05). But they were negatively correlated with APOA level (r value was -0. 459 and -0. 462, respectively, both P<0. 05). Maternal VCAM-1 level was positively correlated with AOPP (r =0. 572,P <0. 05). Conclusion VCAM-1 and AOPP levels in maternal and umbilical cord blood are significantly high, which is closely related with the onset and progression of GDM. Therefore, VCAM-1 and AOPP can be used as monitoring indexes of GDM and its progression.
