目的：通过对本院围手术期预防应用抗菌药物加强干预前后用药合理性的比较，评价加强干预措施的有效可行性，为手术科室抗菌药物的预防应用和管理提供有效方法。方法抽取本院Ⅰ类及Ⅱ类切口手术出院病历，2012年2311份（加强干预前）及2013年2125份（加强干预后），进行加强干预前后围手术期预防用药合理性的对照研究。结果加强干预后，围手术期预防用药在用药指征、药物选择、术前给药时机、给药频次、单次用药剂量、预防用药时间、术前0.5~2h预防用药溶媒、联合用药及术中应用抗菌药物等方面合理性均有显著提高（P<0.001）。结论本院按照三级综合医院等级评审的要求加强了对围手术期预防用药的管理，充分发挥了临床药师对抗菌药物应用的指导作用，使我院围手术期预防用药趋于合理。
목적：통과대본원위수술기예방응용항균약물가강간예전후용약합이성적비교，평개가강간예조시적유효가행성，위수술과실항균약물적예방응용화관리제공유효방법。방법추취본원Ⅰ류급Ⅱ류절구수술출원병력，2012년2311빈（가강간예전）급2013년2125빈（가강간예후），진행가강간예전후위수술기예방용약합이성적대조연구。결과가강간예후，위수술기예방용약재용약지정、약물선택、술전급약시궤、급약빈차、단차용약제량、예방용약시간、술전0.5~2h예방용약용매、연합용약급술중응용항균약물등방면합이성균유현저제고（P<0.001）。결론본원안조삼급종합의원등급평심적요구가강료대위수술기예방용약적관리，충분발휘료림상약사대항균약물응용적지도작용，사아원위수술기예방용약추우합리。
Objective By comparison of the rationality of preventive applying antibiotics during perioperation before and after enhancing the intervention, to judge the possibility of enhancing intervening measure, also to provide effective methods on preventive applying antibiotics in surgical departments.Methods To extract incision cases of 2311 copies in 2012(before intervening) and 2125 cases in 2013(after intervening) which are inⅠ section (cleaning)andⅡ section (clean-pollutional),which is for the comparison research on enhancing the rationality of preventive applying antibiotics during perioperation.Results After enhancing the intervening, the rationality of medication indications, medicine choice, preoperative drug delivery time, frequency, single dosage, prophylactic usage time, preventive medicine solvent 0.5-2h before surgery, drug combination, the application of antibiotics during surgery have been significantly improved(P<0.001). Conclusion We enhance the management of preventive applying antibiotics during perioperation according to requirement of evaluation on the 3A hospital. The guidance of clinical pharmacist in application of antibiotics has worked.The application of antibiotics tends to be rational gradually.
