目的：通过对比研究方法，观察弹拨颈外横突治疗点、针刺两种方法治疗颈源性肩周炎的临床疗效，为颈源性肩周炎的治疗提供简便效捷的适用技术。方法采用随机数字表法将颈源性肩周炎患者126例随机分为观察组和对照组，每组各63例。观察组取颈外横突治疗点进行弹拨；对照组取大椎、阿是穴（压痛点）、夹脊穴（双），患侧新设、大杼、悬钟、中渚、养老、合谷等穴行常规针刺。1次/ d，10次/疗程，两个疗程后行疗效对比分析。观察两组患者治疗前后疼痛强度评分、肩关节功能评分及综合评分的变化情况以及两组间疗效比较。结果两种方法均能有效改善患者症状和体征。观察组治疗前后疼痛强度评分比较（P <0,01），肩关节功能评分比较（P <0,05），差异均有统计学意义。对照组治疗前后疼痛强度评分比较（P <0,05）；肩关节功能评分比较（P <0,05），差异均有统计学意义。治疗组与对照组治疗后疼痛强度评分比较（ P <0,05），差异有统计学意义。观察组总有效率为96,83%，对照组总有效率为90,48%，两组总有效率比较（P >0,05），差异无统计学意义；但两组治愈率（分别为46,03%和20,63%）比较，差异有统计学意义（P <0,05）。结论弹拨颈外横突治疗点与针刺法均能有效缓解疼痛并改善肩关节活动功能。但弹拨颈外横突治疗点在提高临床治愈率和减轻疼痛方面明显优于针刺法。

목적：통과대비연구방법，관찰탄발경외횡돌치료점、침자량충방법치료경원성견주염적림상료효，위경원성견주염적치료제공간편효첩적괄용기술。방법채용수궤수자표법장경원성견주염환자126례수궤분위관찰조화대조조，매조각63례。관찰조취경외횡돌치료점진행탄발；대조조취대추、아시혈（압통점）、협척혈（쌍），환측신설、대저、현종、중저、양로、합곡등혈행상규침자。1차/ d，10차/료정，량개료정후행료효대비분석。관찰량조환자치료전후동통강도평분、견관절공능평분급종합평분적변화정황이급량조간료효비교。결과량충방법균능유효개선환자증상화체정。관찰조치료전후동통강도평분비교（P <0,01），견관절공능평분비교（P <0,05），차이균유통계학의의。대조조치료전후동통강도평분비교（P <0,05）；견관절공능평분비교（P <0,05），차이균유통계학의의。치료조여대조조치료후동통강도평분비교（ P <0,05），차이유통계학의의。관찰조총유효솔위96,83%，대조조총유효솔위90,48%，량조총유효솔비교（P >0,05），차이무통계학의의；단량조치유솔（분별위46,03%화20,63%）비교，차이유통계학의의（P <0,05）。결론탄발경외횡돌치료점여침자법균능유효완해동통병개선견관절활동공능。단탄발경외횡돌치료점재제고림상치유솔화감경동통방면명현우우침자법。
Objective Through comparative research method,this study observed the clinical effica-cy of plucking external carotid transverse process treatment points and two methods of acupuncture treatment in treatment of shoulder periarthritis due to cervical spondylopathy,provides simple and effect for the treat-ment guidance for shoulder periarthritis due to cervical spondylopathy, Methods Using check random num-ber table method,126 patients with shoulder periarthritis due to cervical spondylopathy were randomly divided into observation group and control group,63 cases in each group, Observation group received plucking on ex-ternal carotid transverse process treatmentpoints, Control group received routine acupuncture on dazhui,ashi (pressure point),jiaji( double),xinshe,dazhu,xuanzhong,zhongzhu,yanglao,and hegu point, 1 / d,10 times/ course;curative effect of the two groups were compared after treatment, Pain intensity score before and after treatment in both groups,changes of the shoulder joint function score and comprehensive score and cura-tive effect were compared between the two groups, Results The two methods can effectively improve symp-toms and signs, Pain intensity grading comparison before and after the treatment(P < 0, 01)and the shoulder joint function score comparison(P < 0, 05,differences in the observation group were significant, Pain intensity grading comparison before and after treatment(P < 0, 05)and shoulder joint function score comparison(P <0, 05)were significant of the control group, Treatment group compared with the control group after treatment showed significant difference in pain intensity score(P <0, 05), Observation group total effective rate was 96, 83% ,control group total effective rate was 90, 48% ,total effective rate between the two groups had no signifi-cant difference(P >0, 05), But two groups of the cure rate(46, 03% and 20, 63% ,respectively),demonstratingsignificant difference(P < 0, 05), Conclusion plucking external carotid transverse process treatment points and acupuncture treatment can effectively relieve pain and improve the function of shoulder joint activity, Plucking external carotid transverse process is more effective than acupuncture in improving clinical curative rate and reducing pain.
