目的：探讨全身麻醉联合硬膜外麻醉在胸部手术中的临床应用。方法选取我院在2012年1月～2013年9月期间收治的需要进行肺叶或者肺叶局部切除手术的患者40例，将患者在手术时不同麻醉方式分为实验组和对照组，实验组患者接受全身麻醉联合硬膜外麻醉，对照组患者接受单纯的全身麻醉，在手术前，手术中以及手术后对患者的平卧后收缩压（SBP），心率（HR），舒张压（DBP），平均动脉压（MAP）以及血氧饱和度（SpO2）进行测定并比较；并记录患者在手术后的自主呼吸时间和清醒时间，进行比较。结果对照组患者的各项指标均高于实验组患者，差异具有统计学意义（P <0.05）。实验组患者的清醒时间明显少于对照组患者的清醒时间，差异具有统计学意义（P <0.05）。结论全身麻醉联合硬膜外麻醉应用于胸部手术，能起到保持循环稳定的作用，还能缩短患者的术后清醒时间，效果显著，值得临床推广应用。
목적：탐토전신마취연합경막외마취재흉부수술중적림상응용。방법선취아원재2012년1월～2013년9월기간수치적수요진행폐협혹자폐협국부절제수술적환자40례，장환자재수술시불동마취방식분위실험조화대조조，실험조환자접수전신마취연합경막외마취，대조조환자접수단순적전신마취，재수술전，수술중이급수술후대환자적평와후수축압（SBP），심솔（HR），서장압（DBP），평균동맥압（MAP）이급혈양포화도（SpO2）진행측정병비교；병기록환자재수술후적자주호흡시간화청성시간，진행비교。결과대조조환자적각항지표균고우실험조환자，차이구유통계학의의（P <0.05）。실험조환자적청성시간명현소우대조조환자적청성시간，차이구유통계학의의（P <0.05）。결론전신마취연합경막외마취응용우흉부수술，능기도보지순배은정적작용，환능축단환자적술후청성시간，효과현저，치득림상추엄응용。
Objective Discussion of general anesthesia combined with epidural anesthesia in thoracic surgery clinical application.Methods40 cases in our hospital in January 2012~September 2013 need to be admitted during the partial lung or lobe resection patients, the patient intravenous anesthesia during surgery were divided into experimental group and control group, the experimental group received general anesthesia combined with epidural anesthesia, the control group received general anesthesia alone, before surgery, and after surgery the patient supine systolic blood pressure (SBP), heart rate (HR), diastolic blood pressure (DBP), mean arterial pressure (MAP) and oxygen saturation (SpO2) were measured and compared; and recorded spontaneously breathing patients time and time awake after surgery, were compared.ResultsThe indicators were higher than the control group of patients in the experimental group of patients, the difference was statisticaly significant (P < 0.05). Experimental group of patients with spontaneous breathing time and recovery time was significantly less than the control group of patients with spontaneous breathing time and wake time, and the difference was statisticaly significant (P < 0.05).Conclusion Anesthesia combined with epidural anesthesia applied to the chest surgery, can play a role in maintaining stable circulation, but also shorten the patient's spontaneous breathing time and postoperative recovery time, the effect is significant, worthy of clinical application.
